Indications
Sulprex Inhalation Aerosol is indicated for use in patients with Chronic Obstructive Pulmonary Disease (COPD) who:
Are already on a regular aerosol bronchodilator.
Continue to show evidence of bronchospasm.
Require a second bronchodilator.
Pharmacology and Action
This aerosol is an environmentally friendly, HFA-based inhaler (does not contain CFCs). It contains two bronchodilator drugs that work via different mechanisms to maximize the treatment response:
Salbutamol (B2-adrenergic bronchodilator): A short-acting sympathomimetic that relaxes bronchial smooth muscle.
Ipratropium Bromide (Anticholinergic bronchodilator): Prevents bronchoconstriction caused by acetylcholine binding to muscarinic receptors.
The combination of these two distinct mechanisms (sympathomimetic and anticholinergic) provides a greater bronchodilatory effect than using either drug alone.
Dosage and Administration
Adult Dose (Including the Elderly):2 puffs of Sulprex 4 times a day.
Maximum: Patients may take additional puffs as required, but the total number of puffs should not exceed 12 in 24 hours.
Important Administration Steps:
Shake the inhaler vigorously at least six times before each use.
Breathe out fully, then place the mouthpiece in your mouth and close your lips around it.
While breathing in deeply and slowly through the mouth, press down fully on the canister to release medicine.
Hold your breath for at least 10 seconds (or as long as comfortable).
If more than one puff is prescribed, wait 1 minute between puffs and shake the inhaler again.
Rinse your mouth with water after each treatment to reduce the risk of side effects.
Contraindications and Safety
Contraindications: Hypersensitivity to Soya-Lecithin, related food products (soybean and peanut), or to atropine or its derivatives.
Special Populations Caution: Use with caution in patients with narrow-angle glaucoma, prostatic hypertrophy, or bladder-neck obstruction (due to Ipratropium Bromide). Use caution in patients with convulsive disorders, hyperthyroidism, or diabetes mellitus (due to Salbutamol).
Overdose: Effects are mainly related to Salbutamol and may include anginal pain, hypertension, hypokalemia, and tachycardia.
Side Effects: Common side effects include mild tremor and headache (usually disappear with continued treatment).
Pregnancy & Lactation: Use during pregnancy only if the potential benefit justifies the risk to the fetus. Use cautiously in nursing mothers.
Therapeutic Class: Combined bronchodilators.
