Sitagil 50

Presentation: Each film-coated tablet contains 50 mg of sitagliptin.

Indications: Sitagil 50 is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It maybe used as monotherapy when metformin is inappropriate, or in combination with other antidiabetic agents, including metformin, sulfonylureas, thiazolidinediones, or insulin, to enhance blood glucose control.

Dosage and Administration: The usual dosage is 50 mg once daily, administered orally with or without food.

  • Monotherapy: Typically 100 mg once daily. A dosage of 50 mg once daily may be considered in moderate renal impairment.
  • Combination Therapy: 100 mg once daily when combined with metformin or a thiazolidinedione. When used with a sulfonylurea or insulin, a lower dose of the latter may be warranted to mitigate the risk of hypoglycemia.
  • Renal Impairment:
    • Moderate (CrCl 30to <50mL/min): 50 mg once daily.
    • Severe (CrCl <30mL/min) or End-Stage Renal Disease (requiring hemodialysis or peritoneal dialysis): 25 mg once daily, administered post-dialysis in hemodialysis patients.
  • Hepatic Impairment: No dosage adjustment is generally required.
  • Elderly: Dosage adjustment based solely on age is not typically necessary; however, renal function should be evaluated.

Side Effects: Common adverse reactions may include headache, nausea, upper respiratory tract infection, and nasopharyngitis. An increased incidence of hypoglycemia may occur when Sitagil 50 is used in combination with sulfonylureas or insulin. Less frequent but potentially serious side effects include pancreatitis, hypersensitivity reactions (e.g., rash, pruritus, angioedema), arthralgia, and bullous pemphigoid.

Precautions: Exercise caution in patients with a history of pancreatitis. If pancreatitis is suspected, discontinue Sitagil 50. Patients receiving Sitagil 50 with a sulfonylurea or insulin may require dosage reduction of these agents to minimize hypoglycemic risk. Monitor for signs of bullous pemphigoid and discontinue if it develops. The safety and efficacy in pediatric patients have not been established. Inform healthcare professionals regarding pregnancy and breastfeeding status.

Storage: Store at room temperature (15°C to 30°C), protected from moisture and light. Keep out of reach of children. Do not use after the expiry date.
