Rosumax (Rosuvastatin): A Statin for Lipid Management
Rosumax contains Rosuvastatin, a potent and selective inhibitor of HMG-CoA reductase. It is a statin medication used to manage high cholesterol and reduce the risk of cardiovascular events. Rosuvastatin works by reducing "bad" cholesterol (LDL-C) and triglycerides and increasing "good" cholesterol (HDL-C). It is indicated for various types of hyperlipidemia, including homozygous and heterozygous familial hypercholesterolemia, and for slowing the progression of atherosclerosis.
How Rosumax Works (Pharmacology)
Rosuvastatin produces its lipid-modifying effects through two main mechanisms:
- Inhibits HMG-CoA Reductase: It competitively inhibits HMG-CoA reductase, the key enzyme in the liver that synthesizes cholesterol.
- Increases LDL Receptors: By inhibiting cholesterol synthesis, it increases the number of hepatic LDL receptors on the surface of liver cells. This enhances the uptake and breakdown of LDL, thereby reducing LDL-C levels in the blood.
- Reduces VLDL Synthesis: It also inhibits the synthesis of VLDL (very-low-density lipoprotein), which reduces the total number of VLDL and LDL particles.
Key Indications & Benefits
Rosumax is indicated for:
- Primary hyperlipidemia and mixed dyslipidemia.
- Hypertriglyceridemia.
- Primary dysbetalipoproteinemia.
- Homozygous familial hypercholesterolemia (HoFH) in adults and heterozygous familial hypercholesterolemia (HeFH) in pediatric patients (10-17 years old).
- Slowing the progression of atherosclerosis.
- Reducing the risk of myocardial infarction (MI), stroke, and revascularization procedures in patients with risk factors but without clinically evident coronary heart disease.
Dosage & Administration
The dosage of Rosumax is individualized based on the patient's condition and other medications. Always consult a registered physician for specific dosage instructions.
- Adults (General): The dose should be individualized.
- Very High Cholesterol & High Risk: 40 mg once daily.
- Homozygous Familial Hypercholesterolemia: Start with 20 mg once daily.
- Pediatric Patients (HeFH, 10-17 years): The usual dose is 5-20 mg/day, with a maximum of 20 mg/day.
- Asian Patients: A starting dose of 5 mg once daily should be considered.
- Severe Renal Impairment: Start with 5 mg once daily, and do not exceed 10 mg once daily.
- Concomitant Medications: Dosage adjustments are required for patients taking Cyclosporine, Lopinavir/Ritonavir, or Atazanavir/Ritonavir.

Contraindications:
- Known hypersensitivity to Rosuvastatin.
- Active liver disease or unexplained persistent elevations in liver enzymes.

Side Effects:
- Most frequent: Headache, myalgia (muscle pain), constipation, asthenia (weakness), abdominal pain, and nausea.
- Serious: Myopathy (muscle disease) and rhabdomyolysis, which can lead to renal failure.
Pregnancy & Lactation:
- Pregnancy: It is contraindicated in pregnant women. If pregnancy occurs, the drug should be discontinued immediately.
- Lactation: It is contraindicated in nursing mothers.
Precautions & Warnings:
- Myopathy: Use with caution in patients with predisposing factors for myopathy (e.g., age over 65, hypothyroidism, renal impairment).
- Drug Interactions: The risk of myopathy is increased with concurrent use of other lipid-lowering therapies (Fibrates or Niacin), Gemfibrozil, Cyclosporine, Lopinavir/Ritonavir, or Atazanavir/Ritonavir.
- Liver Function: Liver enzyme tests should be performed before and periodically during treatment.
- Severe Conditions: Rosumax therapy should be temporarily withheld in patients with acute, serious conditions that could suggest myopathy or predispose to rhabdomyolysis.
Overdose Effects
- There is no specific treatment for Rosumax overdose.
- Treatment should be symptomatic and supportive.
- Hemodialysis does not significantly enhance the clearance of Rosumax.
