Miotrol

Presentation: Each film-coated tablet contains 2.5 mg of Zolmitriptan USP.

Indications:Miotrol is indicated for the acute treatment of migraine with or without aura in adults.It is not used to prevent migraine attacks. 

Dosage and Administration:The recommended dose to treat a migraine attack is 2.5 mg.

  • If symptoms persist or return within 24 hours, a second dose has been shown to be effective,but it should not be taken within 2 hours of the initial dose. 
  • If a patient does not achieve satisfactory relief with 2.5 mg doses, subsequent attacks can be treated with5 mg doses. The maximum single dose is 5 mg. 
  • The total intake of Zolmitriptan in a 24-hour period should not exceed 10 mg.
  • Miotrol can be taken with or without food. 
  • It is advisable to take Miotrol tablets as early as possible after the onset of a migraine headache.

Side Effects: Adverse reactions are typically mild to moderate, transient, not serious, and resolve spontaneously without additional treatment. Possible side effects tend to occur within 4 hours of dosing. Common side effects may include:

  • Nausea
  • Dizziness
  • Somnolence (drowsiness)
  • Warm sensation
  • Asthenia (weakness)
  • Dry mouth
  • Neck, throat, or jaw pain or pressure
  • Paresthesia (tingling or numbness)
  • Heaviness or tightness in the throat, neck, limbs, and chest
  • Myalgia (muscle pain)
  • Muscle weakness
  • Feeling of cold
  • Flushing (sense of warmth in the face, ears, neck, and trunk)
  • Tightness sensation

Precautions: Care should be taken to exclude other potentially serious neurological conditions before using Miotrol. It is contraindicated in patients with:

  • Known hypersensitivity to any component of the product.
  • Uncontrolled hypertension.
  • Ischaemic heart disease.
  • Coronary vasospasm / Prinzmetal's angina. 
  • History of cerebrovascular accident (CVA) or transient ischaemic attack (TIA).
  • Symptomatic Wolff-Parkinson-White syndrome or arrhythmias. 

Caution should be exercised in patients with risk factors for coronary artery disease (e.g., hypertension, hypercholesterolemia, smoking, obesity, diabetes, strong family history of ASCVD, menopause, men >40 years of age) and a cardiac evaluation is recommended before use. Miotrol may cause a significant elevation in blood pressure. Sensations of tightness, pain, and pressure may occur in the chest, throat, neck, and jaw after treatment. 

Pregnancy and Lactation: Miotrol is Pregnancy Category C by the FDA. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether Zolmitriptan is excreted in human milk. A decision should be made whether to discontinue nursing orthe drug, considering the importance of the drug to the mother. 

Drug Interactions: Concomitant use of other 5-HT1B/1D agonists within 12 hours of Zolmitriptan treatment should be avoided. It is advised to wait at least 24 hours following the use of ergotamine-containing preparations before administering Zolmitriptan,and at least six hours following the use of Zolmitriptan before administering any ergotamine preparation due to a theoretical increased risk of coronary vasospasm. A maximum intake of 5 mg Zolmitriptan nasal spray in 24 hours is recommended in patients taking an MAO inhibitor or general P450 inhibitor. 

Storage: Protect from light and moisture, store below 30°C. Keep the medicine out of the reach of children.
