Zemicef (Cefixime): A Third-Generation Cephalosporin Antibiotic

Zemicef contains Cefixime, a third-generation semisynthetic cephalosporin antibiotic. It is an orally active antibiotic with broad-spectrum bactericidal activity against a wide variety of Gram-positive and Gram-negative bacteria. It is highly stable in the presence of beta-lactamase enzymes, making it effective against many organisms that are resistant to penicillins and some other cephalosporins. Zemicef is indicated for the treatment of various acute infections of the respiratory and urinary tracts.


 

How Zemicef Works (Pharmacology)

Cefixime works by interfering with the synthesis of the bacterial cell wall:

  • Inhibits Cell Wall Synthesis: It kills bacteria by interfering with the synthesis of the peptidoglycan layer, a crucial component of the bacterial cell wall.
  • Resistant to Beta-Lactamases: Cefixime is highly stable in the presence of beta-lactamase enzymes, which are produced by some bacteria to break down antibiotics like penicillin. This stability allows it to be effective against a broader range of bacteria, including those resistant to other antibiotics.
  • Absorption: Its absorption is about 40% to 50% after oral administration, and it is not significantly affected by food.

 

Key Indications & Benefits

Zemicef is indicated for the treatment of:

  • Upper Respiratory Tract Infections (URTI): Otitis media, tonsillitis, pharyngitis, laryngitis, sinusitis.
  • Lower Respiratory Tract Infections (LRTI): Bronchitis.
  • Urinary Tract Infections (UTI): Cystitis, cystourethritis, pyelonephritis.
  • Typhoid: A specific dosage is recommended for typhoid fever in children.

 

Dosage & Administration

Zemicef is administered orally, and the usual course of treatment is 7 days, which may be extended to 14 days. Always consult a registered physician for specific dosage instructions.

  • Adults and Children over 10 years: 200-400 mg daily, given as a single dose or in two divided doses.
  • Children (6 months to 10 years): The recommended dosage is 8 mg/kg/day, given as a single dose or in two divided doses. For children with typhoid, the dose is 5 mg/kg body weight twice daily for 10-14 days.
  • Elderly: Same dose as adults, but dosage should be adjusted in case of severe renal impairment.
  • Renal Impairment: For patients with creatinine clearance less than 20 ml/min, a dose of 200 mg once daily should not be exceeded.

 

Important Considerations & Warnings

Contraindications:

  • Hypersensitivity to cephalosporin antibiotics.

Side Effects:

  • Most common: Gastrointestinal disturbances, such as diarrhea, nausea, abdominal pain, and flatulence.
  • Hypersensitivity reactions: Rash, itching, urticaria, drug fever, and joint pain.
  • Other: Headache, dizziness, and changes in blood tests (e.g., thrombocytopenia, leukopenia).
  • Rare but serious: Pseudomembranous colitis.

Pregnancy & Lactation:

  • Pregnancy: There are no adequate and well-controlled studies in pregnant women. It should be used during pregnancy only if clearly needed.
  • Lactation: It is not known if Cefixime is excreted in human milk. Caution should be exercised when administering to a nursing mother.

Precautions & Warnings:

  • Hypersensitivity: Zemicef should be used with caution in patients with a history of hypersensitivity to other drugs, especially penicillin, due to the possibility of cross-allergenicity.
  • Renal Impairment: Administer with caution in patients with markedly impaired renal function.
  • Diarrhea: Broad-spectrum antibiotics, including Zemicef, can alter the normal flora of the colon and may lead to antibiotic-associated diarrhea caused by Clostridium difficile.

Drug Interactions:

  • Anticoagulants: Increases in prothrombin times have been noted in patients receiving anticoagulants. Caution should be taken.

 

Overdose Effects

  • There is no specific antidote for Cefixime overdose.
  • It is not removed in significant quantities by hemodialysis or peritoneal dialysis.
  • In case of overdose, gastric lavage may be indicated, and treatment should be supportive.

 

Storage Conditions

Store at a temperature below 30ÂēC, protected from light and moisture. Keep out of the reach of children.
