Vori (Voriconazole): A Triazole Antifungal Medication

Vori contains Voriconazole, a triazole antifungal medicine. It is indicated for treating serious fungal infections in patients 12 years of age and older. These infections include invasive aspergillosis, candidemia, esophageal candidiasis, and serious infections caused by Scedosporium apiospermum and Fusarium species. It is also used for patients who are intolerant of or unresponsive to other therapies. Vori belongs to the therapeutic class of Other Antifungal preparations.


How Vori Works (Pharmacology)

Voriconazole's mechanism of action is to inhibit a key enzyme in the fungal cell membrane:

  • Inhibits Ergosterol Synthesis: It binds to and inhibits CYP450-dependent 14-alpha sterol demethylase, an enzyme crucial for the synthesis of ergosterol.
  • Depletes Ergosterol: Ergosterol is a vital component of the fungal cell membrane. By inhibiting its synthesis, Voriconazole leads to a depletion of ergosterol, which disrupts the cell membrane and ultimately kills the fungus.

Key Indications & Benefits

Vori is indicated for the treatment of:

  • Invasive aspergillosis.
  • Candidemia (in non-neutropenic patients) and disseminated candidiasis in various parts of the body.
  • Esophageal candidiasis.
  • Serious infections caused by Scedosporium apiospermum and Fusarium species.
  • Patients who are intolerant of or refractory to other antifungal treatments.

Dosage & Administration

Vori is available in oral (tablet, suspension) and injectable forms. Oral medication should be taken at least one hour before or after a meal. The dosage varies based on body weight and the route of administration. Always consult a registered physician for specific dosage instructions.

  • Oral (Adults & children over 40 kg):
    • Loading dose: 400 mg every 12 hours for the first 24 hours.
    • Maintenance dose: 200 mg twice daily.
  • Oral (Adults & children under 40 kg):
    • Loading dose: 200 mg every 12 hours for the first 24 hours.
    • Maintenance dose: 100 mg twice daily.
  • Injection (Invasive Aspergillosis):
    • Loading dose: 6 mg/kg every 12 hours for the first 24 hours.
    • Maintenance dose: 4 mg/kg every 12 hours.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Vori.

Contraindications:

  • Known hypersensitivity to Voriconazole or its components.
  • Co-administration with certain drugs due to the risk of serious adverse reactions (e.g., terfenadine, astemizole, pimozide) or loss of efficacy (e.g., rifampin, carbamazepine, St. John's Wort).

Side Effects:

  • Most common: Abdominal pain, anemia, blurred vision, headache, chest pain, nausea, and diarrhea.

Pregnancy & Lactation:

  • Pregnancy: There are no adequate studies in pregnant women. It should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
  • Lactation: It is unknown if Voriconazole is excreted in human milk.

Precautions & Warnings:

  • Long-term treatment: Long-term use (over 180 days) requires a careful assessment of the benefits and risks.
  • Skin Cancer: Squamous cell carcinoma of the skin has been reported in patients on long-term Voriconazole treatment.

Drug Interactions:

  • CYP450 enzymes: Voriconazole interacts with various drugs by affecting the CYP3A4, CYP2C9, and CYP2C19 enzymes. This can increase or decrease the concentration of other drugs.
  • Phenytoin or Efavirenz: The dose of Vori may need to be increased when co-administered with these drugs.

Overdose Effects

  • There is no available data on overdose effects.

Storage Conditions

Store tablets in a dry place below 25°C, protected from light. The powder for suspension should be stored between 2°C to 8°C. Keep all medicine out of the reach of children. The reconstituted suspension should be stored between 15°C and 30°C and discarded after 14 days.
