Unigerd 10 

Presentation: Unigerd 10 Tablet, available in Bangladesh, contains Vonoprazan Fumarate INN equivalent to Vonoprazan 10 mg.

Indications: Unigerd (Vonoprazan) is indicated for the treatment of acid-related disorders including:

  • Gastric ulcer
  • Duodenal ulcer
  • Reflux esophagitis
  • Prevention of recurrent gastric or duodenal ulcer associated with low-dose aspirin administration
  • Prevention of recurrent gastric or duodenal ulcer associated with non-steroidal anti-inflammatory drug (NSAID) administration
  • Adjunct therapy to Helicobacter pylori eradication in specific conditions.

Dosage and Administration: The usual adult dose for oral administration is 10 mg of Vonoprazan once daily for the prevention of recurrence of gastric or duodenal ulcers during low-dose aspirin or NSAID administration. For the treatment of gastric ulcer and duodenal ulcer, the usual adult dosage is 20 mg once daily for up to 8 weeks and 6 weeks, respectively. For reflux esophagitis, the usual adult dosage is 20 mg once daily for 4 weeks, which may be extended up to 8 weeks if needed. For maintenance therapy to prevent recurrence or relapse of reflux esophagitis, the dosage is 10 mg once daily, which may be increased to 20 mg once daily if the response is inadequate. For Helicobacter pylori eradication, the recommended regimen for adults is 20 mg of Vonoprazan, 750 mg of Amoxicillin, and 200 mg of Clarithromycin administered orally at the same time twice daily for 7 days. The dose of clarithromycin may be increased as clinically warranted,not exceeding 400 mg twice daily. Unigerd can be taken without regard to food or the timing of food. Always follow your doctor's specific instructions.

Side Effects: Side effects reported with the use of Vonoprazan (Unigerd) may include diarrhea, constipation, hypersensitivity reactions (including anaphylactic shock and urticaria), hepatotoxicity, jaundice, rash, nausea, abdominal distension, edema, and eosinophilia. Increased levels of gamma-glutamyl transferase (GGT), aspartate aminotransferase (AST), alanine aminotransferase (ALT), and abnormal liver function tests have also been reported.

Precautions: Unigerd (Vonoprazan) is contraindicated in patients with known hypersensitivity to Vonoprazan or any of the excipients and in patients receiving atazanavir sulphate, nelfinavir, or rilpivirine hydrochloride. It should be used in pregnant women or women having the possibility of being pregnant only if the expected therapeutic benefit is thought to outweigh any possible risk.It is advisable to avoid the administration of Vonoprazan to nursing mothers;if administration is indispensable, nursing should be discontinued. Caution is advised when Vonoprazan is co-administered with other sensitive CYP3A4 substrates, notably those having a narrow therapeutic index,such as midazolam. The course of the disease should be closely observed during treatment, and the minimum therapeutic need should be considered. In long-term treatment, close observation by such means as endoscopy should be made. In the maintenance of healing of reflux esophagitis, Unigerd should be administered onlyto patients who repeat recurrence and recrudescence of the condition. Administration to patients who do not necessitatemaintenance of healing should be avoided. When healing is maintained over a long period and when there is no risk of recurrence, dose reduction to 10 mg from 20 mg or suspension of administration should be considered. Symptomatic response to Vonoprazan does not exclude the presence of gastric malignancy. If Clostridium difficile-associated diarrhea, including pseudomembranous colitis, occurs, appropriate measures, including discontinuation oftreatment, should be taken. Avoid use in patients with moderate to severe hepatic impairment (Child-Pugh B or C).

Storage: Do not store above 30°C. Keep away from light and out of the reach of children.
