Topmate 50 Tablet

Presentation: Each film-coated tablet contains 50 mg of Topiramate USP.

Indications: Topmate is indicated for:

• Epilepsy: As adjunctive therapy in adults and children (2-16 years of age) with partial-onset seizures or primary generalized tonic-clonic seizures. As monotherapy in adults and children (10 years and older) with partial-onset seizures or primary generalized tonic-clonic seizures.
• Migraine prophylaxis in adults.

Dosage and Administration: The dosage varies depending on the condition being treated and the patient's age and response. It is usually administered orally in one or two divided doses daily.

• Epilepsy (Adjunctive Therapy):
  • Adults: Initiate at 25-50 mg daily, then titrate to an effective dose (e.g., 200-400 mg/day).
  • Children (2-16 years): Initiate at 1-3 mg/kg/day, then titrate to an effective maintenance dose (e.g., 5-9 mg/kg/day).
• Epilepsy (Monotherapy):
  • Adults: Initiate at 25 mg once daily, then increase gradually to the recommended maintenance dose of 200 mg twice daily.
  • Children (10 years and older): Initiate at 25 mg once daily for the first week, then increase weekly to the recommended maintenance dose of 100 mg twice daily.
• Migraine Prophylaxis: The recommended starting dose is 25 mg once daily, increased weekly by 25 mg to the target dose of 50 mg twice daily.

Side Effects: Common side effects may include:

• Paresthesia (tingling or prickling sensation)
• Fatigue
• Dizziness
• Nervousness
• Difficulty with concentration/attention, memory, and speech
• Nausea
• Weight loss
• Anorexia

Less common but potentially serious side effects can occur, including kidney stones, metabolic acidosis, and vision problems. It's important to discuss any unusual symptoms with your doctor.

Precautions:

• Use with caution in patients with a history of kidney stones, metabolic acidosis, or severe renal impairment.
• May cause hyperchloremic, non-anion gap metabolic acidosis.
• Cognitive/neuropsychiatric adverse reactions are common.
• May cause vision problems, including acute myopia and secondary angle closure glaucoma.
• Gradual withdrawal of Topmate is recommended to minimize the potential for increased seizure frequency.
• Use with caution during pregnancy and breastfeeding; consult your doctor about potential risks.

Contraindications: Topmate is contraindicated in patients with known hypersensitivity to topiramate or any of the excipients.

Drug Interactions: Topmate can interact with other medications, including:

• Other antiepileptic drugs (e.g., phenytoin, carbamazepine, valproic acid)
• Oral contraceptives (may decrease their effectiveness)
• Carbonic anhydrase inhibitors (e.g., acetazolamide)
• Central nervous system depressants (e.g., alcohol, sedatives)
