Thyrolar 50

Presentation: Tablets containing 50 mcg of levothyroxine sodium.

Indications: Thyrolar 50 is a synthetic thyroid hormone (levothyroxine) indicated for use as replacement or supplemental therapy in patients with hypothyroidism of any etiology, except transient hypothyroidism during the recovery phase of subacute thyroiditis.It is also indicated for suppression of pituitary thyroid-stimulating hormone (TSH) in patients with well-differentiated, TSH-dependent thyroid cancer and may be used in the diagnosis of suppressed thyroid function.

Dosage and Administration:The dose of Thyrolar 50 is highly individualized and depends on the patient's age, weight, cardiovascular status, concomitant medical conditions (including pregnancy), and the specific clinical and laboratory response to therapy.The usual starting dose in adults is 50 mcg once daily. However, in patients with known cardiovascular disease, a lower starting dose (e.g., 12.5 or 25 mcg daily) may be warranted.The dose is usually adjusted in 12.5 to 25 mcg increments every 2 to 4 weeks until the desired clinical and biochemical response is achieved. The full replacement dose in adults may be 1.6 to 1.8 mcg per kg per day. Thyrolar 50 tablets should be taken orally once daily, preferably on an empty stomach, at least 30 minutes to one hour before breakfast, to ensure adequate absorption. Follow your doctor's instructions carefully.

Side Effects: When used appropriately, levothyroxine is generally well-tolerated. However, excessive dosage can lead to symptoms of hyperthyroidism, such as palpitations, rapid heart rate, nervousness, irritability, insomnia, tremor, weight loss, increased appetite, heat intolerance, excessive sweating, and diarrhea. Chest pain or worsening of heart problems may also occur.If you experience any of these symptoms, inform your doctor, as your dosage may need to be adjusted.

Precautions:

• Inform your doctor about any pre-existing medical conditions, especially heart disease (angina, coronary artery disease, history of myocardial infarction), high blood pressure, adrenal insufficiency, or diabetes mellitus.
• Initiate levothyroxine therapy at a lower dosage in patients with cardiovascular disease and monitor closely for cardiac adverse effects. 
• Adequate treatment of adrenal insufficiency should precede the initiation of thyroid hormone therapy to prevent an acute adrenal crisis. 
• Changes in thyroid status may affect the control of diabetes mellitus and require adjustments in antidiabetic medication dosage. 
• Certain medications can affect the absorption or metabolism of levothyroxine.Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements, especially antacids, iron supplements, calcium supplements, and certain seizure medications. 
• Thyroid hormone requirements may increase during pregnancy. Regular monitoring of thyroid function tests is essential in pregnant women with hypothyroidism.
• Use with caution in elderly patients, who may be more susceptible to cardiac side effects.
