Sompraz (Esomeprazole): A Proton Pump Inhibitor for Acid-Related Disorders
Sompraz contains Esomeprazole, the S-isomer of omeprazole. It is a proton pump inhibitor (PPI) that effectively suppresses gastric acid secretion. It is used to treat a range of conditions, including chronic heartburn, erosive esophagitis, H. pylori infections, and Zollinger-Ellison Syndrome. It belongs to the therapeutic class of Proton Pump Inhibitors.
How Sompraz Works (Pharmacology)
Esomeprazole provides acid suppression by:
- Inhibiting the Proton Pump: It specifically and irreversibly blocks the H+/K+-ATPase enzyme system, also known as the proton pump, which is the final step in the production of gastric acid by parietal cells in the stomach.
- Superior Acid Control: As the first single optical isomer of a PPI, it offers better acid control compared to racemic proton pump inhibitors.
- Pharmacokinetics: The drug is formulated as enteric-coated pellets to protect it from stomach acid. After oral administration, peak plasma levels are reached in about 1.5 hours.
Key Indications & Benefits
Sompraz is indicated for:
- Chronic heartburn symptoms and other symptoms associated with Gastroesophageal Reflux Disease (GERD).
- Healing of erosive esophagitis.
- Maintenance of healing of erosive esophagitis.
- Eradication of Helicobacter pylori infection (in combination with amoxicillin and clarithromycin) in patients with duodenal ulcer disease.
- Zollinger-Ellison Syndrome.

Dosage & Administration
Sompraz is available in oral and intravenous forms. The dosage and administration method depend on the condition being treated. Always consult a registered physician for specific dosage instructions.
Oral Dosage (Capsules, Tablets, or Suspension):
- Healing of Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks.
- Maintenance of Healing of Erosive Esophagitis: 20 mg once daily.
- Symptomatic GERD: 20 mg once daily for 4 weeks.
- H. pylori eradication (Triple Therapy): Esomeprazole 40 mg once daily for 10 days, in combination with Amoxicillin 1000 mg twice daily and Clarithromycin 500 mg twice daily.
- Zollinger-Ellison Syndrome: 20-80 mg once daily, adjusted individually.
- Acid-related Dyspepsia: 20-40 mg once daily for 2-4 weeks.
- Duodenal ulcer: 20 mg once daily for 2-4 weeks.
- Gastric ulcer: 20-40 mg once daily for 4-8 weeks.
IV Injection or Infusion:
- GERD with Erosive Esophagitis (Adults): 20 mg or 40 mg once daily.
- GERD with Erosive Esophagitis (Pediatric Patients):
- Body weight <55 kg: 10 mg once daily.
- Body weight ≥55 kg: 20 mg once daily.
- 1 month to <1 year: 0.5 mg/kg once daily.
- Risk reduction of rebleeding of Gastric or Duodenal Ulcers (Adults): 80 mg IV infusion over 30 minutes, followed by a continuous infusion of 8 mg/h for a total of 72 hours.
Administration Instructions:
- Tablets/Capsules: Should be swallowed whole and taken at least one hour before a meal. The pellets inside the capsules should not be chewed or crushed.
- Oral Suspension: The contents of a packet should be mixed in 15 ml of water, stirred, and consumed within 30 minutes. The mixture should not be stored.
- IV Injection: Reconstitute the powder with 5 ml of 0.9% Sodium Chloride and administer intravenously over at least 3 minutes. The solution must be used within 12 hours.

Contraindications:
- Patients with known hypersensitivity to Esomeprazole or any component of the formulation.
Side Effects:
- Common adverse events: Headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.
- These side effects are generally similar for both short-term and long-term use.
Pregnancy & Lactation:
- Pregnancy: Animal studies show no teratogenic effects, but there are no adequate studies in pregnant women. It should be used during pregnancy only if clearly needed.
- Lactation: The excretion of esomeprazole in breast milk has not been studied. Breast-feeding should be discontinued if the use of esomeprazole is considered essential.
Precautions & Warnings:
- Gastric Malignancy: Symptomatic relief from Sompraz does not rule out the presence of gastric malignancy.
- Crushing/Chewing Pellets: The pellets within the capsule or suspension packet should not be chewed or crushed, as this will affect their efficacy.
- Antacids: Antacids may be used concurrently with Sompraz.
- Hepatic Insufficiency: No dosage adjustment is needed for mild to moderate hepatic insufficiency. However, in patients with severe hepatic insufficiency, the dose of 20 mg once daily should not be exceeded.
- Renal Insufficiency: No dosage adjustment is required, as less than 1% of the drug is excreted unchanged in the urine.
- Pediatric Use: Safety and effectiveness have not been established in pediatric patients.
Drug Interactions:
- CYP2C19: Sompraz is metabolized by the enzyme CYP2C19 and may interfere with drugs that are also substrates for this enzyme. For example, co-administration with diazepam (a CYP2C19 substrate) has resulted in a significant decrease in its clearance.
- Gastric pH-dependent drugs: Sompraz can interfere with the absorption of drugs where gastric pH is an important factor for bioavailability, such as ketoconazole, iron salts, and digoxin.
- Clarithromycin: Co-administration with clarithromycin (and amoxicillin) results in increased plasma levels of both Sompraz and 14-hydroxyclarithromycin, which is a useful interaction for H. pylori eradication.
- Phenytoin, Warfarin, Quinidine, and Amoxicillin: Clinically significant interactions with these drugs are not expected.
Overdose Effects
- Symptoms: There are no reports of overdose with Sompraz in humans. In animal studies, high doses were lethal, with signs of toxicity including reduced motor activity, tremors, and convulsions.
- Treatment: No specific antidote is known. Treatment should be symptomatic and supportive. Hemodialysis is not effective due to the extensive protein binding of the drug.
