Solivo (Naproxen/Esomeprazole): A Combination Therapy for Inflammatory Conditions
Solivo is a combination tablet containing an immediate-release layer of Esomeprazole Magnesium and an enteric-coated core of Naproxen. It is indicated for the treatment of inflammatory conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. The addition of Esomeprazole helps to decrease the risk of gastric ulcers associated with NSAID use. It belongs to the therapeutic class of drugs for Osteoarthritis, Rheumatoid Arthritis, and Non-steroidal Anti-inflammatory Drugs (NSAIDs).
How Solivo Works (Pharmacology)
Solivo's dual-action mechanism combines the anti-inflammatory effects of an NSAID with the acid-suppressing effects of a proton pump inhibitor:
- Naproxen: An NSAID with analgesic and antipyretic properties. Its mechanism is to inhibit prostaglandin synthesis, which is responsible for mediating inflammation, pain, and fever.
- Esomeprazole: A proton pump inhibitor (PPI) that suppresses gastric acid secretion. It specifically blocks the H+/K+ -ATPase in the gastric parietal cells, blocking the final step of acid production.
- Combined Action: The tablet is designed so that the Esomeprazole is released first in the stomach, providing protection against acid, before the Naproxen core dissolves in the small intestine for systemic absorption. This formulation helps to mitigate the gastrointestinal side effects commonly associated with Naproxen.
Key Indications & Benefits
Solivo is indicated for the relief of signs and symptoms of:
- Osteoarthritis
- Rheumatoid arthritis
- Ankylosing spondylitis
- To decrease the risk of gastric ulcers in patients who are at risk of developing NSAID-associated gastric ulcers.
Dosage & Administration
The dosage should be carefully considered, using the lowest effective dose for the shortest duration. The tablet must not be split, chewed, crushed, or dissolved. It should be taken at least 30 minutes before meals. Always consult a registered physician for specific dosage instructions.
- Rheumatoid Arthritis, Osteoarthritis, and Ankylosing Spondylitis (Adults):
- One tablet twice daily of either:
- 375 mg naproxen/20 mg of esomeprazole; OR
- 500 mg naproxen/20 mg of esomeprazole.
- Juvenile Idiopathic Arthritis (Adolescent Patients 12 years of age and older):
- Weight > 50 kg: One tablet twice daily of either 375 mg naproxen/20 mg of esomeprazole or 500 mg naproxen/20 mg of esomeprazole.
- Weight 38 kg to < 50 kg: One tablet twice daily of 375 mg naproxen/20 mg of esomeprazole.

Contraindications:
- Known hypersensitivity to any component of the tablet or substituted benzimidazoles.
- History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs.
- Use during the peri-operative period of coronary artery bypass graft (CABG) surgery.
- The third trimester of pregnancy.
Side Effects:
- The esomeprazole component is included to decrease the risk of gastrointestinal side effects from Naproxen.
- Naproxen-related: An increased risk of arterial thrombotic events (myocardial infarction or stroke), especially at high doses and with long-term use. Fluid retention, edema, hypertension, and cardiac failure have also been reported.
- Gastrointestinal: The most common adverse events are GI in nature, including nausea, vomiting, diarrhea, constipation, dyspepsia, abdominal pain, and flatulence. Peptic ulcers, perforation, or GI bleeding can occur and may be fatal, particularly in older individuals.
- Exacerbation of pre-existing conditions: Caution is advised for patients with a history of gastrointestinal diseases like ulcerative colitis or Crohn’s disease, as these may be exacerbated.
Pregnancy & Lactation:
- Pregnancy:Contraindicated during the third trimester. In the first and second trimesters, it should only be used if the potential benefit outweighs the potential risk.
- Breast-feeding: Naproxen is excreted in low quantities in breast milk, and it is unknown if esomeprazole is. Therefore, it should not be used during breast-feeding.
- Fertility: The use of NSAIDs like Naproxen may impair female fertility and is not recommended for women attempting to conceive.
Precautions & Warnings:
- Avoid concomitant use of other NSAIDs, including COX-2 selective inhibitors.
- Risk factors for GI complications: Caution is advised for patients with advanced age, a history of ulcers, or those taking other medications that increase bleeding risk (e.g., anticoagulants, corticosteroids).
- Cardiovascular and Cerebrovascular effects: Use with caution in patients with a history of hypertension, congestive heart failure, or other cardiovascular diseases.
- Renal effects: Long-term NSAID use can lead to renal injury. Not recommended for patients with moderate to severe renal impairment (creatinine clearance <30 ml/min).
- Hepatic insufficiency: Monitor patients with mild to moderate hepatic impairment closely. Not recommended for severe hepatic impairment.
- Vitamin B12 absorption: Long-term use of esomeprazole, like other acid-blocking medicines, may reduce the absorption of vitamin B12.
- Infections: Treatment with PPIs may be associated with a slightly increased risk of gastrointestinal infections like Salmonella and Campylobacter.
Drug Interactions:
- ACE inhibitors, diuretics, and beta-blockers: NSAIDs may reduce their antihypertensive effects.
- Warfarin: Concomitant use may increase the risk of bleeding.
- Ketoconazole, iron salts, and digoxin: Esomeprazole may interfere with their absorption.
- High-fat food: May prolong the time to peak plasma concentration of naproxen.
Overdose Effects
- Naproxen overdose: Symptoms may include lethargy, dizziness, epigastric pain, nausea, and vomiting. Severe cases can involve GI bleeding, renal failure, and respiratory depression. There is no specific antidote; treatment is supportive. Hemodialysis is not effective due to high protein binding.
- Esomeprazole overdose: Symptoms are transient and there is no specific antidote.
