Reumacap SR (Indomethacin): A Non-steroidal Anti-inflammatory Drug (NSAID)

Reumacap SR contains Indomethacin, a non-steroidal anti-inflammatory drug (NSAID) with strong anti-inflammatory, analgesic (pain-relieving), and antipyretic (fever-reducing) properties. It's indicated for the relief of pain and inflammation in various rheumatic conditions and other musculoskeletal disorders. It belongs to the therapeutic class of NSAIDs, specifically listed under Drugs for Osteoarthritis and Drugs used for Rheumatoid Arthritis.


How Reumacap SR Works (Pharmacology)

Indomethacin primarily works by:

  • Inhibiting Cyclooxygenase (COX) Enzymes: Like other NSAIDs, it inhibits the enzyme cyclooxygenase, which is crucial for the production of prostaglandins. Prostaglandins are inflammatory mediators that contribute to pain, swelling, and fever.
  • Anti-inflammatory, Analgesic, and Antipyretic Effects: By reducing prostaglandin synthesis, Indomethacin effectively relieves pain, reduces joint swelling and edema, and improves joint mobility in rheumatic diseases.
  • Absorption and Excretion: After oral administration, it's rapidly and completely absorbed, reaching peak plasma levels within 2 hours. Approximately two-thirds is excreted in urine, and the rest in feces, with about 90% eliminated within 24-48 hours.

Key Indications & Benefits

Reumacap SR is indicated for the relief of pain and inflammation in:

  • Rheumatoid arthritis
  • Ankylosing spondylitis

Dosage & Administration

Reumacap SR is available in sustained-release (SR) capsules and standard capsules, with different dosages for each. Always consult a registered physician for specific dosage instructions.

For 75 mg sustained-release capsule:

  • Rheumatoid arthritis, ankylosing spondylitis, and osteoarthritis: The initial dose is 75 mg once daily. If severe, twice daily may be required.
  • Acute attacks of gout: May be controlled with a dose of 75 mg twice daily.
  • Acute non-articular rheumatism (bursitis, synovitis, tendinitis):1-2 sustained-release capsules daily depending on severity.

For 25 mg capsule:

  • Rheumatic diseases:50-200 mg daily in divided doses.
  • Acute gout:150-200 mg daily in divided doses.
  • Dysmenorrhea: Up to 75 mg daily.

Administration of capsules immediately after meals with an antacid, if necessary, can minimize gastrointestinal side effects.


Important Considerations & Warnings

It's crucial to discuss your full medical history with your doctor before taking Reumacap SR.

Contraindications:

  • Patients with ulcer, gastritis, or active ulcerative colitis. Use with caution in patients with a history of these disorders; suppositories might be preferred in such cases.
  • Patients with known hypersensitivity to Indomethacin or in whom aspirin or other NSAIDs precipitate asthma attacks.

Side Effects:

  • Common during early therapy (often transient): Headaches (usually in the morning), mild vertigo. These often disappear with continued use or dose reduction.
  • Gastrointestinal reactions: Nausea, vomiting, diarrhea, epigastric and abdominal pain. These are often due to large doses and usually subside with dose reduction. Taking the capsule immediately after meals with an antacid can minimize these effects.
  • Rare: Mild psychic disturbances (subsided after a few days of treatment).

Pregnancy & Lactation:

  • Not recommended for pregnant women due to insufficient clinical studies at present.
  • Lactation: Use should be avoided unless potential benefits outweigh possible risks.

Precautions & Warnings:

  • Psychiatric problems, epilepsy, or parkinsonism: Use with caution as the drug may aggravate these conditions.
  • Peptic ulcer or gastrointestinal bleeding: Use with caution in patients with a history of these disorders.
  • Neonates: Reumacap SR is not normally given to neonates, except when used to assist closure of a patent ductus arteriosus (a specific heart condition).

Drug Interactions:

Close monitoring is required when Reumacap SR is used with:

  • Anticoagulants: May enhance the activity of anticoagulants, increasing bleeding risk.
  • Lithium: May increase plasma concentration of lithium, leading to toxicity.
  • Diuretics: May interact with the activity of diuretics, potentially reducing their effectiveness.
  • Antihypertensive drugs: May reduce the blood pressure-lowering effect of these medications.
  • Diflunisal: May increase Indomethacin plasma levels and increase the risk of gastrointestinal adverse effects.
  • Sulfonylureas: May increase the hypoglycemic effect of sulfonylureas.

Storage Conditions

Store in a cool and dry place protected from light. Keep out of the reach of children.
