Indications

Ramace tablets are indicated for several cardiovascular conditions:

  • Hypertension: To lower blood pressure, either as a stand-alone treatment or combined with other medications.
  • Congestive heart failure: Used to manage heart failure, often in combination with diuretics.
  • Post-myocardial infarction: For patients who show clinical signs of congestive heart failure in the days following a heart attack.
  • Nephropathy: For treating kidney disease in both diabetic and non-diabetic patients.
  • Cardiovascular risk reduction: To reduce the risk of heart attack, stroke, or cardiovascular death in high-risk individuals, such as those with existing coronary heart disease, a history of stroke, peripheral vascular disease, or diabetes with other risk factors.

 

Pharmacology

Ramipril is an Angiotensin-Converting Enzyme (ACE) inhibitor. Once in the body, it is converted to ramiprilat, which blocks the enzyme that converts angiotensin I to angiotensin II, a powerful vasoconstrictor. This leads to lower levels of angiotensin II, which reduces blood pressure. Ramipril also helps manage heart failure and reduces the risk of major cardiovascular events. It is a long-acting and well-tolerated medication, making it suitable for long-term therapy.


 

Dosage

The dosage of Ramipril should be adjusted based on the patient's tolerance and response.

  • Hypertension: The usual starting dose for adults is 1.25-2.5 mg once daily. The maintenance dose is typically 2.5-20 mg daily, taken as a single dose or in two divided doses.
  • Congestive heart failure after myocardial infarction: Therapy can begin as early as 2 days after a heart attack with an initial dose of 2.5 mg twice daily. The target dose is 5 mg twice daily.
  • Prevention of major cardiovascular events: The recommended dose is 2.5 mg once daily for the first week, increasing to 5 mg once daily for the next three weeks, and then to a maintenance dose of 10 mg once daily.
  • Renal Impairment: Doses must be carefully adjusted. The maximum daily dose is 5 mg for hypertension and 2.5 mg twice daily for heart failure.

 

Administration

Ramipril tablets should be swallowed whole with a sufficient amount of liquid. They should not be chewed or crushed. The absorption of Ramipril is not affected by food, so it can be taken before, during, or after a meal.


 

Interaction

Combining Ramipril with diuretics can cause severe low blood pressure and, with potassium-sparing diuretics, dangerously high potassium levels. It may also increase the concentration of lithium in the blood. Alcohol can worsen the blood pressure-lowering effect. NSAIDs may reduce Ramipril's effectiveness and harm kidney function.


 

Contraindications

Do not use Ramipril if you have:

  • A hypersensitivity to ramipril or any other ACE inhibitor.
  • A history of angioedema.
  • Hemodynamically significant renal artery stenosis.
  • Hypotensive or unstable hemodynamic states.
  • Are pregnant.
  • Are using medications containing aliskiren (if you have diabetes or kidney impairment) or angiotensin II receptor antagonists (if you have diabetic nephropathy).
  • You must not use this medication with sacubitril/valsartan. There must be a wash-out period when switching between these drugs.

 

Side Effects

Ramace is generally well-tolerated. Common side effects include dizziness, headache, fatigue, and weakness. Less common side effects can include symptomatic low blood pressure, cough, nausea, vomiting, diarrhea, and skin rashes. Rare but serious side effects like angioedema, anaphylactic reactions, and high potassium levels have been reported.


 

Pregnancy & Lactation

Ramipril must not be used during pregnancy. If a patient becomes pregnant while taking this medication, it must be stopped immediately due to the risk of harm to the fetus. Ramipril is also not recommended for use while breastfeeding.


 

Precautions & Warnings

Use Ramace with caution in patients with impaired kidney function, high potassium levels, low blood pressure, and impaired liver function.


 

Use in Special Populations

  • Elderly: A lower starting dose of 1.25 mg daily should be considered.
  • Hepatic Impairment: Treatment should be started under close medical supervision, with a maximum daily dose of 2.5 mg.
  • Renal Impairment: A reduced starting dose of 1.25 mg daily is generally recommended, with a maximum daily dose of 5 mg.
  • Patients on diuretics: The diuretic may need to be stopped or its dose reduced for a few days before starting Ramipril to avoid a sharp drop in blood pressure.

 

Overdose Effects

An overdose may cause dangerously low blood pressure, shock, a slow heart rate, electrolyte disturbances, and kidney failure. In such a case, immediate medical management is necessary, including gastric lavage and supportive therapy to correct blood pressure and fluid levels.

Storage Conditions

Store at or below 30°C, protected from light. Keep out of the reach of children. Do not use after the expiration date. This medication should only be dispensed with a registered physician's prescription.
