Povisep (Povidone Iodine): A Broad-Spectrum Antiseptic

 

Povisep contains Povidone Iodine, a complex of iodine and an organic polymer. It is a broad-spectrum antiseptic with rapid microbicidal activity against bacteria, viruses, fungi, and protozoa. Unlike ordinary iodine, the povidone complex provides a slower, more prolonged release of iodine, which reduces toxicity. Povisep is available in multiple formulations—cream, ointment, solution, gargle, mouthwash, surgical scrub, powder, and ophthalmic solution—for various therapeutic and prophylactic uses.


 

How Povisep Works (Pharmacology)

 

Povidone Iodine exerts its antimicrobial effect by slowly releasing elemental iodine:

  • Slow Liberation of Iodine: The povidone polymer acts as a carrier, slowly releasing elemental iodine. This sustained release prolongs its germicidal activity while reducing toxicity compared to elemental iodine.
  • Broad-Spectrum Activity: The liberated iodine rapidly kills a wide range of microorganisms, including Gram-positive and Gram-negative bacteria, fungi, viruses, and protozoa. It is also sporicidal.
  • Non-staining: It is a non-staining microbicide that remains active even in the presence of organic matter like soap, blood, serum, and pus.

 

Key Indications & Benefits

 

Povisep is indicated for:

  • Therapeutic Use: Treatment of primary or secondary topical infections, infected surgical incisions, ulcers, pyodermas, and infected traumatic lesions.
  • Prophylactic Use: Prevention of microbial contamination in burns, cuts, and other lesions; for preoperative and postoperative skin preparation and scrubbing.
  • Oral Hygiene: Treatment of oral and pharyngeal mucosal infections, and for oral hygiene before and after dental surgery.
  • Ophthalmic Use: Symptomatic treatment of dry eye conditions.

 

Dosage & Administration

 

The dosage and method of administration depend on the formulation. Always consult a registered physician for specific instructions.

  • Cream/Ointment: Apply to a cleaned, dried affected area once or twice daily. Can be covered with a bandage.
  • Solution: Apply full strength as a paint, spray, or wet soak.
  • Gargle & Mouthwash: Gargle or rinse with up to 10 mL for 30 seconds, up to four times daily.
  • Surgical Scrub: Used for hand washing and preoperative skin preparation, with specific instructions for lathering and rinsing.
  • Powder: Apply a light dusting to the affected area.
  • Ophthalmic Solution: 1 drop into the conjunctival sac 4 times daily.

 

Important Considerations & Warnings

 

It is crucial to discuss your full medical history with your doctor before using Povisep.

Contraindications:

  • Hypersensitivity to iodine or povidone.
  • Regular use in patients with thyroid disorders (e.g., goitre, Hashimoto's thyroiditis) or those on lithium therapy.
  • Povidone Iodine Powder should not be used in serious body cavities or in children under 2 years of age.

Side Effects:

  • Hypersensitivity reactions and skin irritation.
  • Systemic effects: Application to severe burns or large skin areas may cause metabolic acidosis, hypernatraemia, and impaired renal function.
  • Thyroid function: It may interfere with thyroid function tests.

Pregnancy & Lactation:

  • Not recommended for regular use in pregnancy or lactation due to the potential for iodine absorption to affect the fetal or infant thyroid.

Precautions & Warnings:

  • Consult a physician: For deep or puncture wounds, serious burns, or if redness, irritation, or swelling persists.
  • Avoid "pooling": In preoperative prepping, to prevent skin irritation.
  • Gargle/Mouthwash: Avoid regular or prolonged use (more than 14 days) to prevent significant iodine absorption. Not for children under 6 years.
  • Surgical Scrub: Can permanently discolor white gold jewelry. Use with caution in patients with renal insufficiency.
  • Ophthalmic Solution: Do not wear contact lenses during instillation. Avoid touching the dropper tip to any surface. Discard one month after opening.
  • Internal use: Povidone Iodine Powder is not for internal use.

Drug Interactions:

  • Lithium therapy: Concurrent use may lead to a hypothyroidic effect.
  • Others: Interacts with strong alkalis, sodium thiosulphate, sodium metabisulphite, and thiomersal.

 

Storage Conditions

Store below 25°C, protected from light and moisture. Do not freeze. Keep out of the reach of children.
