Pantogen 20

Presentation:

Pantogen 20 is an enteric-coated tablet containing 20 mg of Pantoprazole (as Pantoprazole Sodium Sesquihydrate) as its active ingredient. It is typically a yellow, oval-shaped tablet. The enteric coating ensures that the tablet bypasses the stomach and dissolves in the intestine, protecting the active ingredient from stomach acid. It is usually supplied in blister packs.

Indications:

Pantogen 20 (Pantoprazole) is a proton pump inhibitor (PPI) indicated for conditions where suppression of gastric acid secretion is beneficial. These include:

  • Gastroesophageal Reflux Disease (GERD): Including symptomatic GERD and for the long-term management and prevention of relapse in reflux esophagitis (inflammation of the esophagus due to acid reflux).
  • Peptic Ulcer Disease: Treatment of gastric ulcers and duodenal ulcers.
  • Eradication of Helicobacter pylori: In combination with appropriate antibiotics, for patients with H. pylori-associated duodenal ulcers.
  • Zollinger-Ellison Syndrome: And other pathological hypersecretory conditions where the stomach produces excessive acid.
  • Prevention of NSAID-associated Gastric or Duodenal Ulcers: In patients at risk who require continuous non-steroidal anti-inflammatory drug (NSAID) treatment.

Dosage and Administration:

The dosage and duration of treatment with Pantogen 20 depend on the specific condition and individual patient response. It is generally recommended to take Pantogen 20 once daily, preferably in the morning, at least one hour before a meal. The tablets should be swallowed whole and not chewed, crushed, or split.

Typical dosages are:

  • For Gastroesophageal Reflux Disease (GERD): 20 mg or 40 mg once daily for 4-8 weeks. For long-term management, a maintenance dose of 20 mg once daily is often recommended.
  • For Gastric Ulcer: 40 mg once daily, typically for 4 to 8 weeks.
  • For Duodenal Ulcer: 40 mg once daily, typically for 2 to 4 weeks.
  • For H. pylori Eradication: 40 mg twice daily in combination with specific antibiotics for a designated period (e.g., one week).
  • For Prevention of NSAID-associated ulcers: 20 mg once daily.
  • For Zollinger-Ellison Syndrome: Initial dosage may be higher and adjusted based on acid output.

Dosage adjustments may be necessary for patients with severe liver impairment. Renal impairment generally does not require dosage adjustment.

Side Effects:

Pantogen 20 is generally well-tolerated, but some side effects may occur, including:

  • Common: Headache, diarrhea, nausea, vomiting, abdominal pain, flatulence, dizziness.
  • Less Common: Dry mouth, rash, itching, insomnia, muscle and joint pain.
  • Rare but Serious: Severe allergic reactions (e.g., swelling of the face, lips, tongue, or throat, difficulty breathing), severe skin reactions (e.g., Stevens-Johnson syndrome), liver problems, kidney problems (e.g., interstitial nephritis), Clostridium difficile-associated diarrhea, and blood disorders.
  • Long-term use (especially high doses): Increased risk of bone fractures (osteoporosis), and vitamin B12 deficiency due to reduced acid-dependent absorption. Low magnesium levels (hypomagnesemia) can also occur.

If you experience any severe or persistent side effects, or any signs of an allergic reaction or pancreatitis (severe abdominal pain), seek medical attention immediately.

Precautions:

Before taking Pantogen 20, inform your doctor if you have:

  • A history of allergic reactions to pantoprazole or other proton pump inhibitors.
  • Severe liver disease.
  • Osteoporosis or risk factors for bone fractures.
  • A history of vitamin B12 deficiency.
  • Any unusual weight loss, vomiting blood, black stools, or difficulty swallowing, as these could be signs of a serious underlying condition.
  • Are pregnant or breastfeeding.

Pantogen 20 may mask the symptoms of gastric malignancy; therefore, the presence of gastric malignancy should be excluded before initiating treatment. Long-term use may increase the risk of certain infections, including Clostridium difficile infection.

Pantogen 20 may interact with certain medications, including:

  • Drugs that require an acidic gastric pH for absorption (e.g., ketoconazole, iron salts, digoxin).
  • Anticoagulants (e.g., warfarin), as pantoprazole may alter their anticoagulant effect.
  • Methotrexate (especially high-dose).
  • Atazanavir and nelfinavir (anti-HIV medications).

Always inform your doctor about all medicines, vitamins, and herbal supplements you are taking. Avoid alcohol consumption while on this medication, as it can aggravate acid-related conditions.

Storage:

Store Pantogen 20 tablets at room temperature (below 30°C), in a dry place, and protected from light. Keep out of the reach and sight of children. Do not use after the expiry date printed on the pack.
