Ovestin

Presentation: Ovestin is available in several forms:

  • Vaginal Cream: Containing 1 mg estriol per gram. It is usually supplied in a tube with an applicator for intravaginal administration.
  • Tablets: Containing estriol, typically available in 1 mg or 2 mg strengths.

Indications: Ovestin, containing the active ingredient estriol (a type of estrogen), is a Hormone Replacement Therapy (HRT) used in postmenopausal women for the relief of symptoms related to estrogen deficiency. These symptoms can include:

  • Vaginal atrophy: Thinning and dryness of the vaginal walls, leading to discomfort, itching, and painful intercourse (dyspareunia).
  • Recurrent vaginal infections.
  • Urinary problems: Such as urinary incontinence and recurrent cystitis.
  • Ovestin vaginal cream may also be used pre- and post-operatively in women undergoing vaginal surgery and as a diagnostic aid in doubtful cervical smears.
  • Ovestin tablets may also be prescribed for other conditions like certain forms of infertility and climacteric complaints such as hot flushes and night sweats.

Dosage and Administration:

  • Ovestin Vaginal Cream: The usual initial dosage for vaginal complaints is one application daily for the first few weeks, preferably at night. The dose is then gradually reduced to a maintenance dose, for example, one application twice a week, based on symptom relief. Follow your doctor's instructions on how to use the applicator.
  • Ovestin Tablets: The dosage varies depending on the condition being treated. For vaginal atrophy, the initial dose is usually 4-8 mg daily for the first few weeks, followed by a gradual reduction to a maintenance dose of 1-2 mg daily. For other indications, the dosage will be determined by your doctor. Tablets should be swallowed with some water or other drink, preferably at the same time every day.

Side Effects:

Common side effects of Ovestin may include:

  • Local irritation or itching in or around the vagina (for the cream).
  • Increased vaginal discharge.
  • Swelling and tenderness of the breasts.
  • Nausea.
  • Fluid retention, possibly leading to swollen ankles or feet.
  • Headache.

Less common but potentially serious side effects are similar to those associated with other HRT treatments, although the risk with low-dose vaginal estriol is generally considered lower due to less systemic absorption. These can include an increased risk of:

  • Endometrial cancer (cancer of the lining of the womb), especially with oral tablets and prolonged use.
  • Breast cancer.
  • Ovarian cancer.
  • Blood clots (venous thromboembolism).
  • Stroke.
  • Heart disease (not recommended for women with existing heart disease).

Precautions:

Do not use Ovestin if you have any of the following conditions:

  • Known, past, or suspected breast cancer.
  • Known or suspected estrogen-dependent malignant tumors.
  • Undiagnosed vaginal bleeding.
  • Untreated endometrial hyperplasia.
  • A history of blood clots.
  • Certain blood clotting disorders.
  • Active or recent arterial thromboembolic disease (e.g., angina, myocardial infarction). 
  • Acute liver disease or a history of liver diseaseuntil liver function tests are normal.
  • Porphyria.
  • Pregnancy or breastfeeding.
  • Allergy to estriol or any of the other ingredients.

Use Ovestin with caution if you have or have had certain conditions, such as uterine fibroids, endometriosis, a family history of estrogen-dependent cancer, high blood pressure, liver or kidney disorders, diabetes, gallstones, migraine, or epilepsy.

It is important to have regular medical check-ups, including breast examinations and mammograms, while using HRT. Report any unexpected vaginal bleeding to your doctor immediately.

Storage: Store below 30°C in a dry place, protected from light. Keep out of reach of children.
