Indications

Mexclav, a combination of Cefuroxime and Clavulanic Acid, is indicated for treating a variety of infections caused by susceptible bacteria. This includes infections that may be resistant to other antibiotics.

  • Upper Respiratory Tract Infections: Pharyngitis, tonsillitis, acute bacterial otitis media, and acute bacterial maxillary sinusitis.
  • Lower Respiratory Tract Infections: Pneumonia and acute bacterial exacerbations of chronic bronchitis.
  • Skin and Soft Tissue Infections: Uncomplicated skin and skin-structure infections.
  • Urinary Tract Infections: Uncomplicated urinary tract infections.
  • Other Infections: Bone and joint infections, uncomplicated gonorrhea, early Lyme disease, septicemia, and meningitis.

Pharmacology

Mexclav works through a two-part mechanism:

  • Cefuroxime: A second-generation cephalosporin antibiotic that kills bacteria by inhibiting the synthesis of their cell walls.
  • Clavulanic Acid: A beta-lactamase inhibitor that protects Cefuroxime from being broken down by enzymes produced by resistant bacteria. This combined action makes it effective against a broad range of Gram-positive and Gram-negative organisms, including many that produce beta-lactamase.

Dosage & Administration

Mexclav tablets can be taken with or without food. The duration of treatment varies, but is typically 5 to 20 days depending on the infection.

  • Adolescents and Adults (13 years and older):
    • Pharyngitis/Tonsillitis: 250 mg twice daily.
    • Acute Bacterial Sinusitis: 250 mg twice daily.
    • Pneumonia/Chronic Bronchitis: 250-500 mg twice daily.
    • Uncomplicated Gonorrhea: 1000 mg as a single dose.
  • Pediatric Patients (3 months to 12 years):
    • Pharyngitis/Tonsillitis: 20 mg/kg/day, divided into two doses.
    • Acute Otitis Media/Sinusitis: 30 mg/kg/day, divided into two doses.

Interaction

  • Probenecid: Co-administration can increase the concentration of Cefuroxime in the blood.
  • Gastric Acidity Reducers: Drugs that reduce stomach acid may lower the absorption of Cefuroxime.

Contraindications

Do not use Mexclav if you have a known allergy to cephalosporin antibiotics or a history of pseudomembranous colitis.

Side Effects

Mexclav is generally well-tolerated. Common side effects include nausea, vomiting, diarrhea, and abdominal pain. Prolonged use may lead to overgrowth of non-susceptible microorganisms. Rarely, allergic reactions such as angioedema, pruritus, rash, and anaphylaxis can occur.

Pregnancy & Lactation

While it's best to avoid all antibiotics in the first trimester if possible, Mexclav can be used safely in the later stages of pregnancy. It is excreted in breast milk in small amounts, so the possibility of sensitizing the infant should be considered.

Precautions & Warnings

Use with caution in patients taking potent diuretics or those with a history of colitis.

Storage Conditions

Store in a cool, dry place below 30°C, away from light and moisture. Keep out of the reach of children.
