Metsa XR 500 

Presentation:Extended-release tablets containing 500mg of metformin hydrochloride.Usually film-coated and available in blister packs. Appearance may vary by manufacturer in Bangladesh (e.g., Synovia Pharma PLC).

Indications:For the treatment of type 2 diabetes mellitus, especially in overweight patients, when diet and exercise alone are insufficient.May be used alone or with other antidiabetic drugs or insulin in adults, and alone or with insulin in children (10 years and older).  

Dosage and Administration: Usually started at 500mg once daily with the evening meal. Dosage may be increased weekly in 500mg increments up to a maximum of 2000mg once daily with the evening meal, or 1000mg twice daily with meals.Swallow whole; do not crush, cut, or chew.Take with meals to reduce gastrointestinal side effects.Follow your doctor's instructions.

Side Effects:Common side effects include nausea, vomiting, diarrhea, abdominal pain, and loss of appetite, especially at the start.Metallic taste may occur. Less common but serious: lactic acidosis (unusual tiredness, muscle pain, breathing issues, stomach pain) and vitamin B12 deficiency with long-term use.  

Precautions:Contraindicated in severe kidney disease (eGFR < 30 mL/min/1.73 m²), metabolic acidosis (including diabetic ketoacidosis), and known metformin hypersensitivity.Use caution in moderate kidney impairment, heart failure, liver disease, and the elderly. Temporary discontinuation may be needed before surgery or certain procedures with iodinated contrast.Avoid excessive alcohol.

Storage:Store below 30°C in a dry place, protected from light and moisture.Keep out of reach of children.
