Metacard MR is a modified-release tablet containing 35 mg of Trimetazidine Dihydrochloride. It is manufactured by Aristopharma Ltd.

Indications: 

Metacard MR is indicated in adults as add-on therapy for the symptomatic treatment of patients with stable angina pectoris who are inadequately controlled by or intolerant to first-line antianginal therapies. 

Pharmacology: 

Trimetazidine Dihydrochloride is a metabolic anti-ischemic agent. It inhibits the fatty acid oxidation pathway by inhibiting the 3-keto acyl CoA thiolase enzyme, leading to a shift towards glucose oxidation. This process is more efficient in producing energy for the heart with the same amount of oxygen and also helps prevent the buildup of lactic acid, which can contribute to angina.

Dosage and Administration:

The recommended dose of Trimetazidine is 35 mg twice daily during meals. The benefit of the treatment should be assessed after three months, and Metacard MR should be discontinued if there is no treatment response.

Contraindications:

Metacard MR is contraindicated in patients with:

  • Hypersensitivity to trimetazidine or any of the excipients.
  • Parkinson's disease, parkinsonian symptoms, tremors, restless legs syndrome, and other movement disorders.
  • Severe renal impairment.

Side Effects:

Metacard MR is generally safe and well-tolerated. Common side effects may include:

  • Dizziness
  • Headache
  • Abdominal pain
  • Diarrhea
  • Dyspepsia (indigestion)
  • Nausea
  • Vomiting
  • Rash
  • Pruritus (itching)
  • Urticaria (hives)
  • Asthenia (weakness)

Pregnancy and Lactation:

As a precautionary measure, it is preferable to avoid the use of Trimetazidine during pregnancy due to a lack of data. It is unknown whether Trimetazidine is excreted in human milk, so breastfeeding is not recommended during treatment.

Precautions and Warnings:

Metacard MR is not a curative treatment for angina attacks, nor is it an initial treatment for unstable angina pectoris or myocardial infarction.

Drug Interactions:

No drug interactions have been reported so far, including with beta-blockers, calcium antagonists, nitrates, heparin, hypolipidemic agents, or digitalis preparations.

Storage Conditions:

Keep in a dry place away from light and heat. Keep out of the reach of children.
