Maxima (Esomeprazole): A Proton Pump Inhibitor (PPI)

Maxima contains Esomeprazole, a proton pump inhibitor (PPI). It is the S-isomer of omeprazole and provides better acid control than racemic PPIs. Maxima is indicated for the relief of chronic heartburn symptoms and other symptoms associated with GERD, the healing and maintenance of healing of erosive esophagitis, and the eradication of Helicobacter pylori infection in patients with duodenal ulcer disease. It is also used for Zollinger-Ellison Syndrome, acid-related dyspepsia, and gastric and duodenal ulcers.

How Maxima Works (Pharmacology)

Esomeprazole's mechanism of action is to specifically inhibit the final stage of gastric acid secretion:

• Inhibits the Proton Pump: It suppresses gastric acid secretion by blocking the H+/K+-ATPase enzyme system in the gastric parietal cells. This enzyme system is the "proton pump."
• Reduces Gastric Acid Production: By inhibiting the proton pump, Maxima effectively stops the production of gastric acid.

Maxima is indicated for:

• Relief of chronic heartburn symptoms and other symptoms of GERD (Gastroesophageal Reflux Disease).
• Healing of erosive esophagitis and maintenance of healing.
• Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease (as part of a triple therapy regimen).
• Zollinger-Ellison Syndrome.

Dosage & Administration

Maxima is available in oral dosage forms (capsules, tablets, and oral suspension) and as an IV injection or infusion. The dosage and duration of treatment vary depending on the condition. Oral doses should be taken one hour before a meal. Always consult a registered physician for specific dosage instructions.

• Healing of Erosive Esophagitis (Adults): 20 mg or 40 mg once daily for 4-8 weeks.
• Symptomatic GERD (Adults): 20 mg once daily for 4 weeks.
• H. pylori Eradication: Triple therapy with Maxima 40 mg once daily for 10 days, Amoxicillin 1000 mg twice daily for 10 days, and Clarithromycin 500 mg twice daily for 10 days.
• IV Injection/Infusion: Doses are used for GERD and to reduce the risk of rebleeding from gastric or duodenal ulcers.

Contraindications:

• Known hypersensitivity to any of the components of the formulation.

Side Effects:

• Most common: Headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.
• These side effects are similar in both short-term and long-term treatment.

Pregnancy & Lactation:

• Pregnancy: There are no adequate and well-controlled studies in pregnant women. It should be used during pregnancy only if clearly needed.
• Lactation: It is not known if esomeprazole is excreted in human milk. Breastfeeding should be discontinued if the use of esomeprazole is considered essential.

Precautions & Warnings:

• Gastric Malignancy: Symptomatic response to Maxima therapy does not rule out the presence of gastric malignancy.
• Difficulty Swallowing: Patients who have difficulty swallowing capsules can mix the pellets from inside the capsule with applesauce and swallow immediately. The pellets should not be chewed or crushed.
• Hepatic Insufficiency: No dosage adjustment is needed for mild to moderate hepatic insufficiency, but a dose of 20 mg once daily should not be exceeded in patients with severe hepatic insufficiency.

Drug Interactions:

• CYP2C19: Maxima can inhibit CYP2C19, the major enzyme that metabolizes it. This may decrease the clearance of drugs that are also substrates of this enzyme, such as diazepam.
• Gastric pH-dependent drugs: Maxima inhibits gastric acid secretion, which may interfere with the absorption of drugs that require an acidic environment to be absorbed (e.g., ketoconazole, iron salts, and digoxin).
• Combination Therapy: When Maxima is used with clarithromycin and amoxicillin, plasma levels of both Maxima and 14-hydroxyclarithromycin may increase.

Overdose Effects

• There have been no reports of overdose with Maxima.
• In the event of an overdose, treatment should be symptomatic and supportive.
• Due to extensive protein binding, Maxima is not expected to be removed by dialysis.
