Linatin M 1000 

Presentation:Linatin M 1000 Tablet, available in Bangladesh, is a combination medicine containing Linagliptin 2.5 mgand Metformin Hydrochloride BP 1000 mg.

Indications:Linatin M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with both linagliptin and metforminis appropriate.This includes patients whose diabetes is not adequately controlled on metformin alone or who are already treated with both linagliptin and metformin as separate tablets.

Dosage and Administration:The dosage of Linatin M should be individualized based on the patient's current treatment regimen, glycemic control, and tolerability, using the lowest effective dose.It is generally administered twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

• Patients not currently treated with metformin: The usual starting dose is one tablet of Linatin M 500 (Linagliptin 2.5 mg / Metformin HCl 500 mg) twice daily with meals. After assessing tolerability and response, the dose may be increased to Linatin M 1000 twice daily if needed.
• Patients switching from metformin monotherapy: The starting dose of Linatin M should provide the same total daily dose of metformin as was previously taken, plus linagliptin 2.5 mg twice daily.
• Patients switching from separate tablets of linagliptin and metformin:The starting dose of Linatin M should be based on the doses of linagliptin and metformin the patient was already taking. 
• Maximum Daily Dose: The maximum recommended daily dose of linagliptin is 5 mg and metformin is 2550 mg. With Linatin M 1000, the maximum recommended dose is one tablet twice daily (linagliptin 5 mg and metformin 2000 mg).
• Administration:The tablet should be swallowed whole and should not be crushed, split, or chewed.  

Side Effects:Common side effects associated with metformin include gastrointestinal disturbances such as diarrhea, nausea, vomiting, andflatulence, particularly during initiation of therapy.Hypoglycemia is not usually seen with metformin or linagliptin monotherapy but can occur when Linatin M is used in combination with insulin secretagogues (e.g., sulfonylureas).Side effects potentially related to linagliptin include nasopharyngitis and headache. Less common side effects may include dizziness and skin rash. Lactic acidosis is a rare but serious side effect associated with metformin.

Precautions: Linatin M is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or a history of serious hypersensitivity reaction to linagliptin or metformin. It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment (eGFR 30-45 mL/min/1.73 m²), heart failure, liver disease, and excessive alcohol intake due to the increased risk of lactic acidosis associated with metformin. Renal function should be assessed prior to initiation of therapy and periodically thereafter. Discontinue Linatin M if pancreatitis is suspected.

Storage:Store Linatin M 1000 tablets in a cool (below 30°C) and dry place, protected from light and moisture.Keep out of reach of children.
