Linaglip-M ER (Linagliptin + Metformin HCl): A Combination for Type 2 Diabetes

 

Linaglip-M ER is a combination oral hypoglycemic preparation containing Linagliptin, a DPP-4 inhibitor, and Metformin Hydrochloride, a biguanide. It is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when both components are an appropriate treatment. The extended-release (ER) formulation allows for once-daily dosing.


 

How Linaglip-M ER Works (Pharmacology)

 

The two active ingredients work through different mechanisms to improve glycemic control:

  • Linagliptin: This component is a dipeptidyl peptidase-4 (DPP-4) inhibitor. It works by preventing the breakdown of incretin hormones (GLP-1 and GIP), which are naturally released after a meal. This leads to increased insulin release from the pancreas in a glucose-dependent manner and decreased glucagon secretion from the liver.
  • Metformin Hydrochloride: As a biguanide, Metformin lowers blood sugar by decreasing hepatic glucose production (the amount of glucose the liver makes), reducing the intestinal absorption of glucose, and improving insulin sensitivity in the body's tissues, which increases the uptake and utilization of glucose.

Linaglip-M ER is indicated for:

  • Improving glycemic control in adults with type 2 diabetes mellitus.

 

Dosage & Administration

 

The dosage of Linaglip-M ER should be individualized based on effectiveness and tolerability. The extended-release tablet should be taken once daily with a meal. The maximum recommended total daily dose is 5 mg of Linagliptin and 2000 mg of Metformin Hydrochloride.

  • Recommended Starting Dose (Metformin-naive patients): 5 mg Linagliptin/1000 mg Metformin ER once daily.
  • Patients on Metformin: Start with 5 mg of Linagliptin total daily dose and a similar total daily dose of Metformin once daily.
  • Patients Switching from IR to ER: Switch to the ER tablet containing the same total daily dose of each component, taken once daily.

 

Important Considerations & Warnings

 

It is crucial to discuss your full medical history with your doctor before taking Linaglip-M ER.

Contraindications:

  • Renal impairment: The risk of metformin accumulation and lactic acidosis increases with renal impairment.
  • Acute or chronic metabolic acidosis, including diabetic ketoacidosis.
  • Hypersensitivity to Linagliptin or Metformin.

Side Effects:

  • Most common: Nasopharyngitis and diarrhea.
  • Hypoglycemia: More common when used with sulfonylureas.
  • Lactic Acidosis: A rare but serious and potentially fatal side effect associated with Metformin. Symptoms include abdominal discomfort, fast breathing, muscle pain, and unusual sleepiness.

Pregnancy & Lactation:

  • There are no adequate and well-controlled studies in pregnant women. It should be used during pregnancy only if clearly needed.
  • Caution should be exercised when administered to a nursing mother.

Precautions & Warnings:

  • Lactic Acidosis: The drug should be discontinued immediately if lactic acidosis is suspected.
  • Pancreatitis: Post-marketing reports of acute pancreatitis exist. Discontinue the drug if pancreatitis is suspected.
  • Medical Procedures: Temporarily discontinue the drug before radiologic studies with iodinated contrast or any surgical procedures requiring food/fluid restriction.
  • Vitamin B12: Metformin may lower Vitamin B12 levels. Hematologic parameters should be monitored annually.
  • Alcohol: Drinking alcohol may cause severe low blood sugar.
  • Hypoglycemia: Be aware of the symptoms of low blood sugar and how to treat it.

Drug Interactions:

  • Cationic drugs: May compete for renal transport systems with Metformin.
  • Carbonic Anhydrase Inhibitors: May increase the risk of lactic acidosis.
  • P-gp and CYP3A4 Inducers (e.g., Rifampin): May reduce Linagliptin's efficacy.
  • Drugs affecting glycemic control (e.g., corticosteroids, diuretics): May cause hyperglycemia or hypoglycemia, requiring close monitoring.

 

Overdose Effects

 

  • Treatment: Supportive measures should be employed.
  • Linagliptin: Not removed by hemodialysis or peritoneal dialysis.
  • Metformin: Is dialyzable. Overdose can lead to lactic acidosis.

 

Storage Conditions

Store in a cool and dry place (below 30°C), protected from light and moisture. Keep out of the reach of children.
