Presentation: Each tablet contains 2 mg of glimepiride and 500 mg of metformin hydrochloride. The tablets are typically oblong and film-coated, with specific markings depending on the manufacturer. Available in blister packs.
Indications: Limpet M is indicated as a second-line treatment for type 2 diabetes mellitus in patients whose hyperglycemia cannot be controlled satisfactorily by diet and exercise plus either metformin or a sulfonylurea alone. It is used to improve glycemic control in adults with type 2 diabetes.
Dosage and Administration: The usual starting dose is one tablet (2 mg glimepiride / 500 mg metformin) once or twice daily with meals. Dosage should be individualized based on the patient's response and glucose control. The maximum daily dose of glimepiride is 8 mg, and the maximum daily dose of metformin is 2000 mg. Dosage adjustments should be made gradually with regular monitoring of blood glucose. It is generally recommended to take the medication with meals to minimize gastrointestinal side effects associated with metformin.
Side Effects: Common side effects may include hypoglycemia (low blood sugar), nausea, vomiting, diarrhea, abdominal pain, and headache. Metformin may also cause loss of appetite and a metallic taste. Less common but potentially serious side effects include lactic acidosis (associated with metformin) and hepatic dysfunction (rarely associated with sulfonylureas). Symptoms of hypoglycemia may include sweating, tremor, anxiety, blurred vision, and confusion.
Precautions: Limpet M is contraindicated in patients with known hypersensitivity to glimepiride, other sulfonylureas, metformin, or any of the excipients, as well as in patients with severe renal impairment, metabolic acidosis, diabetic ketoacidosis, and acute or chronic metabolic acidosis. Use with caution in patients with moderate renal impairment, heart failure, and liver disease. Alcohol consumption should be avoided due to the increased risk of hypoglycemia and lactic acidosis. Temporary discontinuation of metformin may be necessary in situations of dehydration, severe infection, or before major surgery. Regular monitoring of renal function is recommended. Not recommended for use during pregnancy or breastfeeding.
Storage: Store at a temperature not exceeding 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
