Itracon (Itraconazole): A Triazole Antifungal Agent

Itracon contains Itraconazole, an orally active triazole antifungal agent. It is indicated for the treatment of a wide range of fungal infections, including both superficial and systemic infections. These include oropharyngeal and vulvovaginal candidiasis, various tinea infections, onychomycosis, histoplasmosis, and invasive or disseminated systemic infections like aspergillosis and cryptococcosis. Some formulations use SUBA (Super Bio-available) technology to improve absorption. Itracon belongs to the therapeutic class of drugs for subcutaneous and mycoses.


How Itracon Works (Pharmacology)

Itraconazole's mechanism of action is to inhibit the synthesis of a critical component of fungal cell membranes:

  • Inhibits Cytochrome P-450 Enzymes: It inhibits fungal cytochrome P-450 dependent enzymes.
  • Blocks Ergosterol Synthesis: This enzyme inhibition leads to impaired biosynthesis of ergosterol, which is a major and essential component of the cell membranes of yeast and fungal cells.
  • Disrupts Cell Function: The lack of ergosterol causes a cascade of abnormalities, including increased membrane permeability, changes in enzyme activity, and inhibition of growth, ultimately leading to cell death.

Key Indications & Benefits

Itracon is indicated for the treatment of:

  • Superficial fungal infections: Vulvovaginal candidiasis, pityriasis versicolor, tinea corporis, tinea cruris, tinea pedis, tinea manuum, onychomycosis, and oropharyngeal candidiasis.
  • Systemic fungal infections: Aspergillosis, histoplasmosis, cryptococcosis (including meningitis), and systemic candidiasis.
  • Maintenance in AIDS patients and prophylaxis in neutropenia.

Dosage & Administration

Itracon is available in different strengths (100mg, 200mg, 65mg, 130mg). The dosage and duration of treatment vary significantly based on the type and severity of the fungal infection. It should be administered after a full meal. Always consult a registered physician for specific dosage instructions.

  • Vulvovaginal Candidiasis: 200 mg twice daily for one day (for the 100/200mg formulation).
  • Tinea Corporis/Cruris: 100 mg once daily for 15 days or 200 mg once daily for 7 days.
  • Onychomycosis (Fingernails): 200 mg twice daily for 7 days, with two courses repeated after a 21-day interval.
  • Systemic Aspergillosis: 200 mg once daily for 2-5 months, with the dose increased to twice daily for invasive or disseminated disease.
  • Pediatric Use: The efficacy and safety in pediatric patients have not been established.

Important Considerations & Warnings

Contraindications:

  • Known hypersensitivity to the drug or any ingredient.
  • Severe hepatic disease.
  • Pregnancy.
  • Concomitant use with drugs such as terfenadine, astemizole, cisapride, statins, oral midazolam, or triazolam.

Side Effects:

  • Common: Nausea, abdominal pain, dyspepsia, constipation, headache, dizziness.
  • Less common: Raised liver enzymes, menstrual disorders, allergic reactions (rash, pruritus), hepatitis, and cholestatic jaundice.
  • Rare: Peripheral neuropathy and Stevens-Johnson syndrome.
  • On prolonged use: Hypokalemia, edema, and hair loss.

Pregnancy & Lactation:

  • Pregnancy: Itraconazole is contraindicated in pregnancy.
  • Lactation: Breastfeeding is not recommended while receiving Itraconazole.

Precautions & Warnings:

  • Reduced Gastric Acidity: Absorption of itraconazole is impaired when gastric acidity is reduced. Acid-neutralizing medicines (e.g., aluminum hydroxide) should be taken at least two hours after Itracon.
  • Liver Function: Cases of hepatitis and jaundice have been reported, especially with long-term use. Liver function should be monitored in patients receiving continuous treatment for more than one month.

Drug Interactions:

  • Contraindicated drugs: Terfenadine, astemizole, cisapride, HMG-CoA reductase inhibitors (simvastatin), oral midazolam, or triazolam.
  • Other significant interactions: With rifampin, phenytoin, phenobarbital, digoxin, and calcium channel blockers.
