Glucolin IV 

Presentation: A clear, sterile solution for intravenous infusion containing Dextrose (Glucose) in various concentrations (e.g., 5%, 10%, 25%). Available in different volume containers (e.g., 250 ml, 500 ml, 1000 ml) as bags or bottles, manufactured by Popular Pharmaceuticals Ltd. and other pharmaceutical companies in Bangladesh.

Indications: Glucolin IV Infusion is indicated for:

• Treatment of carbohydrate and fluid depletion.
• Management of hypoglycemic coma.
• Treatment of cerebral edema.
• Management of shock and circulatory collapse.
• Correction of hyperkalemia (when used with insulin).
• As a vehicle for the intravenous administration of compatible medications.
• Providing a readily metabolizable nutrient source for patients unable to take adequate oral intake.

Dosage and Administration: The dosage and rate of infusion depend on the patient's age, weight, clinical condition, fluid and electrolyte balance, and the specific concentration of the Glucolin IV solution. It must be administered intravenously by a trained healthcare professional.

• Fluid replacement: Dosage varies based on individual needs. For 5% dextrose solution, the usual range for adults is 500 to 3000 ml per 24 hours.
• Hypoglycemia: Adults: 10-25 g (40-100 ml of 25% solution or 20-50 ml of 50% solution). Children: Dosage adjusted by weight.
• Hyperkalemia: Adults: 25-50 g combined with insulin, administered over 30-60 minutes. Children: Dosage adjusted by weight.

The infusion rate should be controlled to avoid hyperglycemia and fluid overload. Hypertonic solutions (10% and above) are preferably administered via a central vein.

Side Effects: Potential side effects may include:

• Fluid overload (hypervolemia).
• Electrolyte imbalances (hypokalemia, hyponatremia, hypophosphatemia, hypomagnesemia).
• Hyperglycemia.
• Local reactions at the infusion site (phlebitis, thrombosis, pain, infection).
• Dehydration (with hyperosmolar solutions if not administered properly).
• Allergic reactions.

Precautions:

• Contraindicated in patients with known hypersensitivity to dextrose, severe dehydration, anuria, hepatic coma, intracranial or intraspinal hemorrhage. Concentrated solutions are contraindicated in Glucose-Galactose Malabsorption Syndrome.
• Use with caution in patients with diabetes mellitus, cardiac insufficiency, renal impairment, and those receiving corticosteroids or corticotropin.
• Monitor blood glucose and electrolyte levels during infusion.
• Avoid rapid infusion, especially of hypertonic solutions.
• Ensure compatibility when used as a vehicle for other drugs.

Storage: Store below 30°C, away from light and moisture. Do not use if the solution is cloudy, contains particulate matter, or if the container is damaged. Keep out of reach of children.
