Glipita M XR 1000 

Presentation:Each extended-release tablet of Glipita M XR 1000 contains Sitagliptin Phosphate Monohydrate INN equivalent to Sitagliptin 50 mg and Metformin Hydrochloride BP 1000 mg.It is available in strips.The tablets are formulated for a prolonged release of the medication. The specific shape and markings may vary by manufacturer (Beximco Pharmaceuticals Ltd.) in Bangladesh.

Indications: Glipita M XR is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus whentreatment with both sitagliptin and extended-release metformin is appropriate.  

Dosage and Administration: The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommendeddaily dose of sitagliptin 100 mg and metformin 2000 mg. Usually administered once daily with a meal, preferably in the evening. Initiate with lower doses to minimize gastrointestinal side effects of metformin.

  • Starting dose in metformin-naïve patients: Usually 50 mg sitagliptin/500 mg metformin hydrochloride extended-release once daily, with gradual dose escalation recommended.
  • Switching from metformin: Start with Glipita M XR containing 50 mg sitagliptin and the current total daily dose of metformin as extended-release tablets, taken once daily. For patients on metformin 850 mg twice daily, the starting dose is 50 mg sitagliptin/1000 mg metformin hydrochloride extended-release once daily.
  • Swallow whole with water; do not crush, cut, or chew.  

Side Effects:Common side effects may include diarrhea, nausea, vomiting, upset stomach, and headache.Hypoglycemia may occur when used with insulin or sulfonylureas. Other possible side effects include nasopharyngitis and abdominal pain.

Precautions:Contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or known hypersensitivity to sitagliptin or metformin.Not recommended in moderate renal impairment (eGFR 30-45 mL/min/1.73 m²) unless benefits outweigh risks. Assess renal function before initiation and annually thereafter. Avoid in hepatic disease and excessive alcohol intake. Discontinue if pancreatitis is suspected.

Storage:Store below 30°C in a dry place, protected from light and moisture.Keep out of reach of children.
