Indications

Genac (Diclofenac Sodium) is a non-steroidal anti-inflammatory drug (NSAID) used to treat various inflammatory and painful conditions, including:

• Rheumatology: Inflammatory and degenerative forms of rheumatism, osteoarthritis, and ankylosing spondylarthritis. It is also used for acute gout and peri-articular rheumatic disorders.
• Surgery and Traumatology: Sprains, bruises, dislocations, and fractures.
• Gynecology: Primary dysmenorrhea, adnexitis, endometritis, etc.
• ENT and Dentistry: Pre- and post-operative pain, inflammation, and swelling.
• Other: Renal and biliary colic, and pain and inflammation following urogenital tract operations.

Pharmacology

Diclofenac is a potent NSAID with anti-rheumatic, anti-inflammatory, analgesic, and antipyretic properties. It works by inhibiting prostaglandin biosynthesis, which plays a key role in causing inflammation, pain, and fever. It is rapidly absorbed from the gastrointestinal tract and is highly protein-bound in the plasma.

Dosage & Administration

• FC Tablet (Adults): 75-150 mg daily in 2-3 divided doses, preferably after food.
• SR Tablet (Adults): 1 tablet daily, taken whole with liquid, preferably at mealtimes.
• DT Tablet (Adults): 2-3 tablets daily, with a maximum of 150 mg. It should be dispersed in half a glass of water before swallowing.
• TR Capsule: One capsule daily, preferably after mealtimes.
• Suppository (Adults): 50 mg, 2-3 times daily, with a maximum daily dose of 150 mg.
• Injection (Adults): Usual dose is 1 ampoule daily. May be increased to 2 ampoules in serious cases.
• Gel (External Use): Apply 2-4 g to the affected area 3-4 times daily.
• Children: 1-3 mg/kg body weight daily in divided doses for juvenile rheumatoid arthritis. Not generally recommended for other indications.

Interaction

• Lithium and Digoxin: May increase their plasma concentrations.
• Anticoagulants: May increase the risk of hemorrhage.
• Cyclosporin: Increased risk of nephrotoxicity.
• Methotrexate: Increased risk of serious toxicity if taken within 24 hours.
• Quinolone Antimicrobials: Concomitant use may lead to convulsions.
• Other NSAIDs and Steroids: May increase the frequency of unwanted effects.

Contraindications

Genac is contraindicated in patients with hypersensitivity to any of its ingredients, as well as those with peptic ulcers. It should not be used in asthmatic patients in whom asthma attacks, urticaria, or acute rhinitis are triggered by aspirin or other NSAIDs. The gel should not be used under occlusive dressings.

Side Effects

Common side effects are generally mild and include epigastric pain, nausea, diarrhea, dizziness, and headache. Peripheral edema and skin reactions like rash may also occur. Genac Gel can cause local skin irritation and redness.

Pregnancy & Lactation

During pregnancy, Diclofenac should be used only if absolutely necessary and at the lowest effective dose. It is not recommended during the first trimester. A very small quantity may be detected in breast milk, but is not expected to cause undesirable effects in the infant.

Precautions & Warnings

• Use with caution in elderly patients and those with a history of peptic ulcers or gastrointestinal bleeding.
• The gel should not contact eyes or mucous membranes.
• Patients with advanced age should be closely monitored.
