Freegest (Montelukast): A Leukotriene Receptor Antagonist

Freegest contains Montelukast, a selective and orally active leukotriene receptor antagonist. It works by blocking the action of cysteinyl leukotrienes, which are inflammatory mediators involved in the pathophysiology of asthma and allergic rhinitis. Freegest is indicated for the prophylaxis and chronic treatment of asthma, the prevention of exercise-induced bronchoconstriction, and the relief of symptoms of allergic rhinitis.

How Freegest Works (Pharmacology)

Montelukast's mechanism of action is based on blocking leukotriene receptors:

• Inhibits Leukotriene Receptors: Montelukast selectively inhibits the cysteinyl leukotriene receptor (CysLT1).
• Blocks Inflammatory Effects: By blocking these receptors, it counteracts the effects of cysteinyl leukotrienes (LTC4, LTD4, LTE4). These leukotrienes are released from cells during allergic and inflammatory responses and cause airway edema, smooth muscle contraction, and inflammation, which are key symptoms of asthma and allergic rhinitis.

Dosage & Administration

• Pediatric patients 2 to 5 years: 4 mg tablet once daily.
• Pediatric patients 6 months to 5 years: 4 mg oral granules once daily.
• Hepatic/Renal Insufficiency: No dosage adjustment is required for mild-to-moderate hepatic insufficiency or renal insufficiency.
• Elderly: No dosage adjustment is required.

Contraindications:

Side Effects:

Pregnancy & Lactation:

Precautions & Warnings:

• Acute Asthma Attacks: Freegest is not indicated for the reversal of bronchospasm in acute asthma attacks. Patients should have appropriate rescue medication available.
• Corticosteroids: Freegest should not be abruptly substituted for inhaled or oral corticosteroids.
• Aspirin Sensitivity: Patients with known aspirin sensitivity should continue to avoid aspirin and other NSAIDs while taking Freegest.
• Neuropsychiatric Events: Rare cases of neuropsychiatric events, including agitation, aggression, and suicidal thoughts, have been reported.

Drug Interactions:

• Phenobarbital or Rifampin: Potent cytochrome P450 enzyme inducers may decrease the concentration of Montelukast. Appropriate clinical monitoring is recommended if these drugs are co-administered.

Overdose Effects

• Symptoms: Abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity.
• Treatment: The usual supportive measures should be employed, such as removing unabsorbed material and clinical monitoring.
