Flagyl IV 

Presentation: Flagyl IV is a sterile solution of metronidazole for intravenous infusion. It is typically available as a 500 mg/100 ml solution in a glass vial or a flexible plastic container.

Indications: Flagyl IV (Metronidazole) is an antibiotic and antiprotozoal medication used to treat serious infections caused by susceptible anaerobic bacteria and certain protozoa. These infections may include:

  • Intra-abdominal infections (e.g., peritonitis, abscesses)
  • Skin and soft tissue infections
  • Gynecological infections (e.g., endometritis, tubo-ovarian abscess)
  • Bacterial septicemia
  • Bone and joint infections (as adjunctive therapy)
  • Central nervous system infections (e.g., meningitis, brain abscess)
  • Lower respiratory tract infections (e.g., pneumonia, lung abscess, empyema)
  • Endocarditis
  • Severe Clostridium difficile infections (when oral medication is not feasible)
  • Amoebic dysentery

Flagyl IV may also be used for surgical prophylaxis to prevent postoperative infections, particularly in surgeries involving the gastrointestinal tract.

Dosage and Administration: Flagyl IV is administered by intravenous infusion, usually over 30-60 minutes, under the supervision of a healthcare professional. The dosage and duration of treatment depend on the type and severity of the infection, as wellas the patient's age, weight, and clinical condition.

  • Adults: The typical loading dose is 15 mg/kg infused over one hour. The maintenance dose is usually 7.5 mg/kg infused over one hour every 6 hours, not to exceed 4 g per day.
  • Children: The dosage is usually calculated based on body weight and may vary depending on the age and the infection being treated.
  • Surgical Prophylaxis: A single dose of 15 mg/kg is usually administered intravenously, completed approximately 1 hour before surgery. Maintenance doses may be given at 6 and 12 hours after the initial dose and should be discontinued within 12 hours after surgery.
  • Severe Hepatic Impairment: The dose of Flagyl IV should be reduced by 50% in patients with severe liver impairment.
  • Renal Impairment: Dosage adjustment is usually not necessary in mild to moderate renal impairment. In severe renal impairment or end-stage renal disease, monitoring for adverse events is recommended. Supplementation after hemodialysis may be needed.

The intravenous solution should be prepared and administered according to established hospital procedures. Flagyl IV should be infused separately from other medications if possible.

Side Effects: Common side effects of Flagyl (Metronidazole) may include:

  • Nausea, vomiting
  • Diarrhoea, constipation
  • Abdominal discomfort, cramps
  • Headache, dizziness
  • Metallic taste
  • Dry mouth
  • Darkening of urine

Less common but potentially serious side effects may include:

  • Peripheral neuropathy (numbness, tingling in extremities)
  • Seizures
  • Encephalopathy (confusion, altered mental status)
  • Aseptic meningitis
  • Optic neuropathy
  • Severe allergic reactions (rash, itching, swelling, difficulty breathing)
  • Liver problems (jaundice, abdominal pain)
  • Blood disorders (leukopenia, neutropenia)

The infusion site may experience local reactions such as pain, redness, or swelling.

Precautions: Flagyl IV is contraindicated in patients with known hypersensitivity to metronidazole or other nitroimidazole derivatives. It is also contraindicated in patients withCockayne syndrome due to the risk of severe irreversible hepatotoxicity/acute liver failure.

Caution should be exercised in patients with:

  • Active central nervous system disorders (e.g., epilepsy)
  • Severe liver disease (dosage adjustment is required)
  • Renal impairment (monitoring for adverse events may be needed)
  • History of blood dyscrasias

Alcohol should be avoided during Flagyl IV treatment and for at least 24-48 hours after the last dose due to the possibility of a disulfiram-like reaction (nausea, vomiting, flushing, headache).

Flagyl may interact with other medications, including warfarin (increased risk of bleeding), phenytoin and phenobarbital (decreased metronidazole levels), lithium (increased lithium levels), and certain chemotherapy drugs. Inform your doctor about all medications you are currently taking.

Use in Pregnancy and Lactation: Metronidazole crosses the placenta and its safety in pregnancy, particularly in the first trimester, has not been definitively established. It should be used during pregnancy only if clearly needed and the potential benefits outweigh the risks. Metronidazole is excreted in breast milk in concentrations similar to those found in maternal plasma. Breastfeeding is not recommended during treatment with Flagyl IV, and should be avoided for 12-24 hours after the last dose.

Storage: Flagyl IV infusion should be stored at room temperature (15-25°C) and protected from light. Do not freeze. Once the bag or vial is opened, the solution should be used promptly. Any unused portion should be discarded.
