Epilim 300 

Presentation:Epilim 300 Tablet, available in Bangladesh, typically contains a combination of Sodium Valproate 199.8 mg and Valproic Acid 87 mg, which together are equivalent to Sodium Valproate 300 mg.It is usually presented as a prolonged-release (or controlled-release) oral tablet, often white or off-white in color, and is available in blister packs.The "Chrono" designation in some formulations (like Epilim Chrono) indicates its controlled-release properties, ensuring a slower release of the active ingredient.

Indications: Epilim is indicated for the treatment of various forms of epilepsy in adults and children, including:

  • Generalized seizures (absence, myoclonic, tonic-clonic, atonic, mixed).
  • Partial seizures (simple or complex, secondary generalized seizures).
  • It may also be used for the treatment of mania associated with bipolar disorder in adults, and sometimes for migraine prophylaxis.

Dosage and Administration: The dosage of Epilim must be individualized and is determined by the physician based on the patient's condition, age, body weight, and response to treatment.

  • Adults: The usual starting dose is 600 mg daily, which may be increased gradually by 200 mg every 3 days until seizure control is achieved. The typical maintenance dose is 1000-2000 mg daily (20-30 mg/kg body weight), and in some cases, it can be increased up to 2500 mg per day.
  • Children over 20 kg: The initial dose is 400 mg/day, with gradual increases until control is achieved, usually within the range of 20-30 mg/kg body weight per day.
  • Children under 20 kg: The recommended dose is 20 mg/kg of body weight per day. Syrup formulations might be preferred for this group to allow for more accurate dosing.
  • Epilim Chrono tablets are designed for once or twice daily administration and should be swallowed whole, not crushed or chewed. Taking it with food may help reduce gastrointestinal side effects.
  • When switching from other antiepileptic drugs to Epilim, the previous medication should be tapered off slowly while gradually increasing the dose of Epilim.

Side Effects: Common side effects may include nausea, vomiting, stomach ache, diarrhea, headache, tremor, drowsiness, and hair loss. Changes in appetite and weight (increase or decrease) can also occur.

Less common but potentially serious side effects include:

  • Liver problems (rare but can be severe, especially in young children with certain metabolic disorders). Symptoms may include jaundice, loss of appetite, nausea, vomiting, and fatigue.
  • Pancreatitis (severe stomach pain that may spread to the back).
  • Changes in blood clotting, leading to easy bruising or bleeding.
  • High ammonia levels in the blood (hyperammonemia), which can cause confusion or altered mental status.
  • Severe skin reactions.
  • Suicidal thoughts or behavior (as with other antiepileptic drugs).

Precautions: Epilim is contraindicated in patients with:

  • Active liver disease or a personal or family history of severe hepatic dysfunction, especially drug-related.  
  • Hypersensitivity to sodium valproate orvalproic acid.
  • Porphyria.
  • Known urea cycle disorders.
  • Mitochondrial disorders (e.g., Alpers-Huttenlocher syndrome), particularly in young children.

Caution should be exercised in patients with a history of liver disease, kidney problems, bleeding disorders, and during pregnancy and breastfeeding. Valproate can cause serious birth defects if used during pregnancy, especially in the first trimester. Women of childbearing potential should use effective contraception and discuss alternative treatments with their doctor. Regular monitoring of liver function, blood counts, and valproate levels may be necessary during treatment. Alcohol consumption should be avoided as it can increase the risk of liver problems and may affect seizure control.

Storage: Store Epilim 300 tablets in a cool (below 25°C), dry place, protected from light and moisture. Keep out of reach of children. Do not use after the expiry date stated on the packaging.
