Epilim

Presentation:
Available in multiple formulations:

  • Tablets: 200 mg, 500 mg Sodium Valproate

  • Syrup: 200 mg/5 ml

  • CR Tablets (controlled-release): 300 mg, 500 mg

  • Injection: 400 mg powder for solution

Indications:

  • Treatment of epilepsy (all seizure types including absence, tonic-clonic, myoclonic)

  • Prophylaxis of migraine

  • Bipolar disorder maintenance therapy

Dosage & Administration:

  • Adults (Epilepsy): Start with 600 mg/day in 2 doses (max 2500 mg/day)

  • Children: 20-30 mg/kg/day in 2 divided doses

  • Migraine prophylaxis: 500 mg–1.5 g/day

  • Take with food to reduce GI irritation

  • CR tablets: Swallow whole (do not crush)

Side Effects:

  • Common:

    • Nausea, vomiting

    • Weight gain

    • Tremor

    • Hair loss (usually temporary)

  • Serious (require immediate medical attention):

    • Liver toxicity (dark urine, jaundice)

    • Pancreatitis (severe abdominal pain)

    • Thrombocytopenia (unusual bruising/bleeding)

Precautions:

  • Contraindicated in:

    • Active liver disease

    • Porphyria

    • Known mitochondrial disorders

  • High-risk groups:

    • Children <3 years (higher hepatotoxicity risk)

    • Pregnancy (high teratogenicity risk – absolute avoidance recommended)

    • Elderly (monitor for drowsiness/ataxia)

  • Essential monitoring:

    • LFTs (baseline and regularly)

    • CBC (for blood disorders)

    • Serum ammonia levels (if encephalopathy suspected)

Storage:

  • Store below 30°C

  • Syrup: Use within 3 months of opening

  • Protect from moisture

Important Note:

  • Pregnancy Warning: High risk of neural tube defects (up to 10%) – contraindicated. Women of childbearing age must use effective contraception.

  • Therapeutic Drug Monitoring: Maintain serum levels 50-100 mg/L for epilepsy

Drug Interactions:

  • Dangerous interactions:

    • Carbapenem antibiotics (reduces valproate levels)

    • Lamotrigine (increases severe skin reaction risk)

  • Caution with:

    • Warfarin (alters INR)

    • Phenobarbital (changes metabolism)
