Duzela 20 Capsule contains 20 mg of Duloxetine Hydrochloride.

Indications: Duzela is indicated for the treatment of:

• Major Depressive Disorder (MDD) in adults.
• Generalized Anxiety Disorder (GAD) in adults.
• Diabetic Peripheral Neuropathic Pain (DPNP) in adults.
• Chronic Musculoskeletal Pain in adults.

Pharmacology: Duloxetine is a selective serotonin and norepinephrine reuptake inhibitor (SNRI). It works by helping to restore the balance of these neurotransmitters in the brain, which can improve mood, anxiety, and reduce pain signals.

Dosage and Administration: The dosage is individualized and should be determined by your doctor. Some general guidelines:

• Major Depressive Disorder: The usual starting dose is 60 mg once daily. Some patients may benefit from a starting dose of 30 mg once daily for one week before increasing to 60 mg once daily. The maximum recommended dose is 120 mg per day.
• Generalized Anxiety Disorder: The usual starting dose is 60 mg once daily. Some patients may benefit from a starting dose of 30 mg once daily for one week before increasing to 60 mg once daily. The maximum recommended dose is 120 mg per day.
• Diabetic Peripheral Neuropathic Pain: The recommended dose is 60 mg once daily.
• Chronic Musculoskeletal Pain: The usual starting dose is 30 mg once daily for one week before increasing to 60 mg once daily. The maximum recommended dose is 120 mg per day.

Duzela capsules should be swallowed whole and should not be crushed or chewed. It can be taken with or without food.

Contraindications: Duzela is contraindicated in patients with:

• Known hypersensitivity to duloxetine or any of the excipients.
• Uncontrolled narrow-angle glaucoma.
• Use with monoamine oxidase inhibitors (MAOIs) intended to treat psychiatric disorders or within 14 days of discontinuing an MAOI. Also, an MAOI should not be started within at least 5 days after stopping duloxetine.

Precautions:

• Suicidal Thoughts and Behaviors:Monitor all patients being treated with antidepressants for any indication for clinical worsening and emergence of suicidalthoughts and behaviors, especially during the initial few months of drug therapy and when the dose is adjusted.  
• Serotonin Syndrome: The development of a potentially life-threatening serotonin syndrome has been reported with SNRIs, including duloxetine, particularly with concomitant use of other serotonergic drugs.
• Increased Risk of Bleeding: SNRIs, including duloxetine, may increase the risk of bleeding events.
• Activation of Mania/Hypomania: Use with caution in patients with a history of mania.
• Elevations in Blood Pressure: Monitor blood pressure prior to initiating treatment and periodically throughout therapy.
• Hepatic Impairment: Avoid use in patients with substantial alcohol use or evidence of chronic liver disease.
• Withdrawal Reactions: Discontinuation of duloxetine can result in withdrawal symptoms. Gradual reduction of the dose is recommended.

Side Effects: Common side effects may include:

• Nausea
• Dry mouth
• Headache
• Fatigue
• Somnolence (drowsiness)
• Insomnia (difficulty sleeping)
• Dizziness
• Constipation
• Diarrhea
• Decreased appetite
• Increased sweating

Less common but potentially serious side effects can occur. Report any unusual or severe symptoms to your doctor immediately.

Drug Interactions: Duloxetine can interact with various medications. Inform your doctor about all the medicines you are currently taking, including prescription drugs, over-the-counter medications, and herbal supplements. Significant interactions can occur with MAOIs, other serotonergic drugs, CYP1A2 inhibitors, and drugs that affect blood clotting.

Storage: Store below 30°C, protected from light and moisture. Keep out of reach of children.
