Diretic 20 mg + 50 mg: A Combination Diuretic for Fluid Management

Diretic 20 mg + 50 mg is a fixed-dose combination tablet containing Furosemide (20 mg) and Spironolactone (50 mg). This combination leverages the different yet complementary mechanisms of a loop diuretic (Furosemide) and a potassium-sparing diuretic/aldosterone antagonist (Spironolactone) to achieve effective fluid removal while minimizing potassium loss.


How Diretic Works (Pharmacology)

  • Furosemide (Loop Diuretic): Furosemide acts in the ascending Loop of Henle, inhibiting the Na+/K+/2Cl- co-transporter. This blocks the reabsorption of sodium, potassium, and chloride ions, significantly increasing the excretion of sodium and water in the urine. This action typically leads to potassium loss.
  • Spironolactone (Potassium-Sparing Diuretic & Aldosterone Antagonist): Spironolactone works in the distal tubule by antagonizing the action of aldosterone. This inhibits the reabsorption of sodium in exchange for potassium, thereby promoting sodium excretion and reducing the excessive potassium loss induced by Furosemide.

When combined, these two diuretics produce an additive or synergistic diuretic effect.


Key Indications & Benefits

The Furosemide & Spironolactone combination in Diretic is indicated for the management of conditions associated with fluid retention and/or hypertension, including:

  • Essential hypertension (high blood pressure)
  • Chronic congestive heart failure
  • Hepatic cirrhosis, with collection of fluid in the abdominal cavity (ascites)
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism (a condition characterized by excessive aldosterone production)
  • Resistant edema associated with secondary hyperaldosteronism

Dosage & Administration

The dosage of Diretic is individualized based on patient response.

  • Furosemide 20 mg and Spironolactone 50 mg (e.g., Diretic 20+50): 1 to 4 tablets daily (equivalent to 20 to 80 mg of Furosemide and 50 to 200 mg of Spironolactone) according to the patient’s response.
  • Furosemide 40 mg and Spironolactone 50 mg (for higher doses of both): For previously stabilized patients requiring a higher dosage, one to two tablets daily (Furosemide 40 to 80 mg and Spironolactone 50 to 100 mg).

Special Populations:

  • Children: Not suitable for use in children.
  • Elderly: Both Spironolactone and Furosemide may be excreted more slowly in the elderly.

Important Considerations & Warnings

Contraindications:

  • Patients with anuria (absence of urine formation).
  • Acute renal insufficiency or rapidly deteriorating/severe impairment of renal function (creatinine clearance <30 ml/min).
  • Hyperkalemia (high potassium levels in the blood).
  • Addison's disease (adrenal insufficiency).
  • Patients who are hypersensitive to Spironolactone, Furosemide, or sulphonamides.

Side Effects:

  • Spironolactone-related: Headache, drowsiness, gastrointestinal distress (including cramp and diarrhea), ataxia (lack of coordination), mental confusion, skin rashes.Gynaecomastia (breast enlargement in males) is not uncommon and may persist in rare cases. Other endocrine disorders: hirsutism (excessive hair growth), deepening of the voice, menstrual irregularities, and impotence. May cause hyponatremia (low sodium) and hyperkalemia (high potassium).
  • General/Combination-related: Transient increase in blood-urea-nitrogen concentrations may occur. Mild acidosis has been reported. Excessive diuresis can lead to dehydration and reduction in blood volume with circulatory collapse, and potentially vascular thrombosis and embolism, especially in elderly patients. Serious depletion of potassium and magnesium may lead to cardiac arrhythmias.

Interactions:

  • ACE inhibitors or potassium salts: Increased risk of hyperkalemia.
  • Cardiac glycosides (e.g., Digoxin): Spironolactone increases digoxin levels, potentially leading to digitalis toxicity.
  • Corticosteroids: May cause hypokalemia (low potassium) when used with Spironolactone.
  • Indomethacin and other NSAIDs: May reduce or abolish the blood pressure lowering and diuretic effects of Furosemide.
  • Aminoglycoside antibiotics: Furosemide may increase their ototoxicity (ear damage).
  • Sucralfate: Simultaneous administration with Furosemide may reduce Furosemide's natriuretic (sodium excretion) and anti-hypertensive effect.

Pregnancy & Lactation:

  • Pregnancy: Spironolactone and its metabolites may cross the placental barrier. Use in pregnant women requires careful consideration of benefits versus risks to mother and fetus. Animal studies suggest Furosemide may cause fetal abnormalities. Therefore, Furosemide should only be used in women of childbearing age if appropriate contraceptive measures are taken or if potential benefits justify risks to the fetus.
  • Lactation: Metabolites of Spironolactone have been detected in breast milk. If Spironolactone use is essential, an alternative method of infant feeding should be considered. Furosemide is excreted in breast milk, and breast-feeding should be discontinued if treatment is essential.

Precautions & Warnings:

  • Caution in patients prone to electrolyte deficiency.
  • Use with caution in patients with diabetes,enlarged prostate,hypotension (low blood pressure), and hypovolemia (low blood volume).
