Cefotil Plus 250 is an orally active antibiotic indicated for the treatment of various infections caused by sensitive bacteria. It is particularly effective against infections where the bacteria may produce beta-lactamase enzymes, which can inactivate other antibiotics.
How Cefotil Plus 250 Works (Pharmacology)
Cefotil Plus 250 is a combination medication containing Cefuroxime and Clavulanic Acid, which work together to eliminate bacterial infections:
- Cefuroxime: This is a bactericidal second-generation cephalosporin antibiotic. It kills bacteria by inhibiting bacterial cell wall synthesis, specifically by interfering with the transpeptidation process. Cefuroxime is active against a wide range of Gram-positive and Gram-negative susceptible organisms, including many beta-lactamase producing strains.
- Clavulanic Acid: This component is a naturally derived beta-lactamase inhibitor. It has a similar structure to beta-lactam antibiotics and binds irreversibly to beta-lactamase enzymes, inactivating them. This provides protection to Cefuroxime from degradation by these enzymes, making Cefotil Plus effective for bacterial infections caused by beta-lactam resistant bacteria.

Dosage & Administration
Cefuroxime-Clavulanic Acid tablets (Cefotil Plus 250) may be taken without regard to food. It is important to complete the full course of treatment as prescribed by your doctor, even if you start feeling better, to prevent the infection from returning or becoming harder to treat.
Adolescents and Adults (13 years and older):
- Pharyngitis/Tonsillitis: 250 mg twice daily for 5-10 days.
- Acute Bacterial Maxillary Sinusitis: 250 mg twice daily for 10 days.
- Acute Bacterial Exacerbation of Chronic Bronchitis: 250-500 mg twice daily for 10 days.
- Secondary Bacterial Infections of Acute Bronchitis: 250-500 mg twice daily for 5-10 days.
- Uncomplicated Skin and Skin Structure Infections: 250-500 mg twice daily for 10 days.
- Uncomplicated Urinary Tract Infections: 250 mg twice daily for 7-10 days.
- Uncomplicated Gonorrhoea: 1000 mg twice daily as a single dose.
- Community-Acquired Pneumonia: 250-500 mg twice daily for 5-10 days.
- MDR Typhoid Fever: 500 mg twice daily for 10-14 days.
- Early Lyme Disease: 500 mg twice daily for 20 days.
Paediatric Patients (3 months to 12 years):
- Pharyngitis/Tonsillitis: 20 mg/kg/day twice daily for 5-10 days.
- Acute Otitis Media: 30 mg/kg/day twice daily for 10 days.
- Acute Bacterial Maxillary Sinusitis: 30 mg/kg/day twice daily for 10 days.
- Impetigo: 30 mg/kg/day twice daily for 10 days.

Contraindications:
- Patients with known allergy to cephalosporin antibiotics.
- Patients with Pseudomembranous Colitis.
Side Effects:
Cefotil Plus 250 is generally well tolerated. However, some side effects may occur:
- Gastrointestinal: Nausea, vomiting, diarrhea, abdominal discomfort or pain. Diarrhea may occur as a side effect; taking probiotics may help. Talk to your doctor if you notice bloody stools or develop abdominal cramps.
- Other: Rarely (<0.2%) renal dysfunction, anaphylaxis (severe allergic reaction), angioedema (swelling under the skin), pruritus (itching), rash, and serum sickness-like urticaria (hives).
- Superinfection: As with other broad-spectrum antibiotics, prolonged administration may result in the overgrowth of nonsusceptible microorganisms.
Interactions:
- Probenecid: Concomitant administration with Cefuroxime-Clavulanic Acid increases the area under the serum concentration versus time curve by 50%.
- Drugs that reduce gastric acidity (e.g., antacids, H2-receptor blockers): May result in a lower bioavailability of Cefuroxime and tend to cancel the effect of postprandial absorption.
- Alcohol: Interaction with alcohol is unknown. It is advisable to consult your doctor before consumption.
Pregnancy & Lactation:
- Pregnancy: While all antibiotics should ideally be avoided in the first trimester, Cefuroxime-Clavulanic Acid can be safely used in later pregnancy to treat urinary and other infections. However, it is recommended to use this medicine only after consulting your doctor.
- Lactation: Cefuroxime-Clavulanic Acid is excreted into breast milk in small quantities. It is considered safe to use during breastfeeding, but the possibility of sensitizing the infant should be kept in mind. Always consult your doctor.
Precautions & Warnings:
- Hypersensitivity: Cefuroxime should be given with care to patients who have shown hypersensitivity to other drugs. Cephalosporins should be given with caution to penicillin-sensitive patients, as there is some evidence of partial cross-allergenicity between penicillins and cephalosporins. Patients have had severe reactions (including anaphylaxis) to both classes of drugs. If an allergic effect occurs with Cefotil Plus 250, the drug should be discontinued immediately, and the patient treated with appropriate agents if necessary.
- Diuretics: Cefuroxime should be given with care to patients receiving concurrent treatment with potent diuretics.
- History of Colitis: Use with care in patients who have a history of colitis.
- General:
- Do not skip any doses and finish the full course of treatment even if you feel better. Stopping it early may make the infection come back and harder to treat.
- Stop taking Cefotil Plus 250 and inform your doctor immediately if you develop an itchy rash, swelling of the face, throat or tongue, or breathing difficulties while taking it.
- Do not use leftover medicine for treating any infection in the future. Always consult your doctor before taking any antibiotic.
