Cef-3: An Oral Cephalosporin Antibiotic

Cef-3 is an orally active, third-generation cephalosporin antibiotic with strong bactericidal activity against a broad spectrum of Gram-positive and Gram-negative organisms. It is indicated for the treatment of acute infections caused by susceptible microorganisms.


How Cef-3 Works (Pharmacology)

Cef-3 contains Cefixime, a semisynthetic cephalosporin antibiotic. It works by interfering with the synthesis of the bacterial cell wall, which leads to the death of the bacteria. Cefixime is particularly effective because it is highly stable in the presence of Beta-lactamase enzymes, which are produced by some bacteria to resist other antibiotics like penicillins and some cephalosporins. This stability means Cefixime can effectively treat infections caused by these resistant strains. Approximately 40% to 50% of an oral dose is absorbed, irrespective of food intake.


Key Indications & Benefits

Cef-3 is used to treat various acute infections, including:

  • Upper Respiratory Tract Infections (URTI): Such as otitis media (middle ear infection), and other URTI where the causative organism is known or suspected to be resistant to common antibiotics, or where treatment failure carries significant risk.
  • Lower Respiratory Tract Infections (LRTI): Including bronchitis.
  • Urinary Tract Infections (UTI): Such as cystitis, cystourethritis, and pyelonephritis.

It is effective against common pathogens including Streptococcus pneumoniae, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (both beta-lactamase positive and negative), Moraxella catarrhalis (both beta-lactamase positive and negative), and Enterobacter species.


Dosage & Administration

The absorption of Cefixime is not significantly affected by food. The usual treatment duration is 7 days, but it can be extended up to 14 days if needed.

  • Adults and children over 10 years: The recommended adult dosage is 200-400 mg daily, based on infection severity, given as a single dose or in two divided doses.
  • Elderly: Can receive the same dose as adults, but renal function should be assessed, and dosage adjusted for severe renal impairment.
  • Children: The recommended dosage for children is 8 mg/kg/day, administered as a single dose or in two divided doses.
    • Suspension Dosing Guide:
      • 6 months up to 1 year: 3.75 ml daily
      • 1-4 years: 5 ml daily
      • 5-10 years: 10 ml daily
    • For Typhoid: The recommended children's dose is 5 mg/kg body weight twice daily for 10-14 days.
    • Children weighing more than 50 kg or older than 10 years: Should receive the adult dose (200-400 mg daily).
    • Children less than 6 months: Safety and efficacy have not been established.
  • Dosage in Renal Impairment:
    • Creatinine clearance 20 ml/min: Normal dose and schedule.
    • Creatinine clearance <20 ml/min: Do not exceed 200 mg once daily.
    • Patients on chronic ambulatory peritoneal dialysis or hemodialysis: Same recommendation as for creatinine clearance <20 ml/min.

Important Considerations & Warnings

Contraindications:

  • Known hypersensitivity to cephalosporin antibiotics.

Side Effects: Cef-3 is generally well tolerated. Most adverse reactions are mild and self-limiting.

  • Gastrointestinal disturbances (most frequent): Diarrhea and stool changes (more common with higher doses), nausea, abdominal pain, dyspepsia, vomiting, and flatulence.
  • Pseudomembranous colitis has been reported.
  • Central Nervous System: Headache and dizziness.
  • Hypersensitivity reactions (rare): Rash, pruritus (itching), urticaria (hives), drug fever, and arthralgia (joint pain). These usually resolve upon discontinuation.
  • Hematological and Clinical Chemistry (infrequent and reversible): Thrombocytopenia (low platelets), leukopenia (low white blood cells), and eosinophilia (increased eosinophils). Mild, transient changes in liver and renal function tests.
  • Miscellaneous: Genital pruritus and vaginitis.

Interactions:

  • Anticoagulants (e.g., Warfarin): Increases in prothrombin times have been noted. Use with caution in patients on anticoagulants, and monitoring may be required.

Pregnancy & Lactation:

  • Pregnancy: No adequate and well-controlled studies in pregnant women. Use only if clearly needed and considered essential by the physician.
  • Lactation: It is unknown if Cefixime is excreted in human milk. Caution should be exercised, and use in nursing mothers is generally not recommended unless considered essential by the physician.

Precautions & Warnings:

  • Hypersensitivity: Use with caution in patients with hypersensitivity to other drugs.
  • Penicillin Sensitivity: Administer with caution to penicillin-sensitive patients due to potential cross-allergenicity. Severe reactions (including anaphylaxis) have occurred with both drug classes. If an allergic reaction to Cef-3 occurs, discontinue the drug and treat as necessary.
  • Renal Impairment: Administer with caution in patients with significantly impaired renal function; dosage adjustments are crucial.
  • Antibiotic-Associated Diarrhea: Broad-spectrum antibiotics can disrupt normal colon flora, leading to overgrowth of Clostridium difficile and associated diarrhea.

Overdose Effects:

  • In case of overdose, gastric lavage may be indicated. There is no specific antidote.
  • Cef-3 is not significantly removed by hemodialysis or peritoneal dialysis.
  • Adverse reactions from single doses up to 2 g in healthy volunteers were similar to those seen at recommended doses.

Storage Conditions

Keep below 30ºC temperature, protected from light & moisture. Keep out of the reach of children.
