Bisopro (Bisoprolol Fumarate)

Indications Bisopro, a highly selective β1​ blocker, is indicated for the treatment of:

• Hypertension (High blood pressure)
• Angina (Chest pain)
• Moderate to severe heart failure
• Note: It is not recommended for the emergency treatment of hypertensive crises.

Pharmacology Bisoprolol is the most selective β1​ blocker, primarily reducing heart rate and cardiac output by blocking β1​ adrenergic receptors in the heart. This action decreases arterial hypertension. It is well-absorbed, has an elimination half-life of 10−12 hours, and is metabolized and eliminated equally by the liver and kidneys, supporting once-daily dosing.

Dosage & Administration (Adults) Dosing must be individualized.

• Starting Dose: 5 mg once daily.
• Titration: If the response is inadequate, the dose may be increased to 10 mg and then to 20 mg once daily, with an appropriate interval of 2 weeks between increases.
• Special Populations: The initial dose for patients with renal (creatinine clearance <40 mL/min) or hepatic impairment should also be 5 mg, and dose titration requires caution due to potential accumulation.

Contraindications Bisopro is contraindicated in patients with:

• Cardiogenic shock or overt heart failure.
• Second or third-degree A-V block.
• Sinus bradycardia.

Side Effects Common side effects include fatigue, dizziness, headache, GI disturbances (nausea, diarrhea), cold or numb extremities, and muscle weakness. Bronchospasm may occur in people with asthma or COPD.

Interactions Do not combine with other β-blocking agents. Use caution with certain Calcium Channel Blockers (Verapamil, Diltiazem) and Antiarrhythmic Agents (Disopyramide), as combined use can lead to severe hypotension and cardiac failure.
