Pharmacology Bisoprolol is the most selective β1 blocker, primarily reducing heart rate and cardiac output by blocking β1 adrenergic receptors in the heart. This action decreases arterial hypertension. It is well-absorbed, has an elimination half-life of 10−12 hours, and is metabolized and eliminated equally by the liver and kidneys, supporting once-daily dosing.

Side Effects Common side effects include fatigue, dizziness, headache, GI disturbances (nausea, diarrhea), cold or numb extremities, and muscle weakness. Bronchospasm may occur in people with asthma or COPD.
Interactions Do not combine with other β-blocking agents. Use caution with certain Calcium Channel Blockers (Verapamil, Diltiazem) and Antiarrhythmic Agents (Disopyramide), as combined use can lead to severe hypotension and cardiac failure.
Therapeutic Class: Beta-adrenoceptor blocking drugs.
