Azin 500 is a film-coated tablet containing Azithromycin USP 500 mg. It is manufactured by The ACME Laboratories Ltd. in Bangladesh. 

Indications:

Azin 500 is a broad-spectrum antibiotic used to treat various bacterial infections, including: 

  • Upper Respiratory Tract Infections (URTIs): Such as pharyngitis, tonsillitis, sinusitis, and otitis media (ear infections). 
  • Lower Respiratory Tract Infections (LRTIs): Including bronchitis and pneumonia. 
  • Skin and Soft Tissue Infections.
  • Sexually Transmitted Diseases: For uncomplicated genital infections due to Chlamydia trachomatis in men and women. 
  • Typhoid Fever.

Pharmacology:

Azithromycin is a macrolide antibiotic of the azalide subclass. It works by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting microbial protein synthesis. This action prevents bacteria from growing and multiplying. 

Dosage and Administration:

Azin 500 can be taken with or without food, but taking it with food may help reduce stomach upset. Antacids containing aluminum or magnesium should not be taken within 2 hours of taking Azin 500. 

  • Adults:
    • For most infections (URTIs, LRTIs, Skin & Soft Tissue Infections): 500 mg once daily for 3 days. Alternatively, 500 mg once on day 1, followed by 250 mg once daily for the next 4 days. 
    • For Sexually Transmitted Diseases (Chlamydia trachomatis): 1 gram (1000 mg) given as a single dose. 
    • For Typhoid Fever: 500 mg once daily for 7 days. 
  • Children (over 6 months of age): The usual dose is 10 mg/kg body weight once daily for 3 days. Dosage varies by body weight for suspension formulations. 

Contraindications:

Azin 500 is contraindicated in patients with: 

  • Known hypersensitivity to azithromycin, erythromycin, any macrolide, or ketolide antibiotic. 
  • Liver disease (as the liver is the principal route of excretion). 
  • Coadministration with ergot derivatives. 

Side Effects:

Azin 500 is generally well tolerated. Most side effects are mild to moderate and often gastrointestinal in origin: 

  • Common: Nausea, vomiting, diarrhea, abdominal discomfort (pain/cramps), flatulence, headache, dizziness. 
  • Less common/Rare (but serious): Allergic reactions (rash, itching, swelling, shortness of breath), angioedema, anaphylaxis (rare but serious), severe and persistent diarrhea (could indicate Clostridium difficile infection), abnormal heart rhythms (especially in patients with prolonged QT interval), liver problems (jaundice, hepatitis), muscle weakness (myasthenia gravis exacerbation). 

Precautions and Warnings:

  • Complete the full course of treatment as prescribed by your doctor, even if you feel better, to prevent recurrence and antibiotic resistance. 
  • Inform your doctor if you have heart problems, liver or kidney problems, myasthenia gravis, or low levels of potassium or magnesium in your blood. 
  • Avoid alcohol consumption while taking Azin 500. 
  • Avoid prolonged exposure to sunlight or tanning beds as it can increase sunburn risk. 
  • Rare cases of severe allergic reactions (including angioedema and anaphylaxis) have been reported, some requiring prolonged observation. 
  • May cause transient mild reductions in neutrophil counts. 

Pregnancy and Lactation:

  • Pregnancy: Generally considered safe to use during pregnancy if prescribed by a doctor, with limited safety data available. Consult your doctor to weigh benefits against risks. 
  • Lactation: Azithromycin passes into breast milk. Consult your doctor before breastfeeding while on this medication. 

Drug Interactions:

  • Antacids (containing aluminum or magnesium): May reduce azithromycin absorption; take Azin 500 at least 1 hour before or 2 hours after antacids. 
  • Warfarin and other anticoagulants: May increase prothrombin time; monitor INR closely. 
  • Cyclosporine, Digoxin: Azithromycin can increase plasma levels of these drugs. 
  • Ergot alkaloids (e.g., ergotamine): Concomitant use is contraindicated due to potential for ergotism. 
  • Other macrolide antibiotics: Enhanced side effects. 

Storage Conditions:

Store in a cool and dry place below 30°C, protected from light and moisture. Keep out of reach of children.
