Indications
Amdocal (amlodipine besilate 5 mg) treats essential hypertension, chronic stable angina, and confirmed/suspected vasospastic angina. It may be used alone or with other antihypertensive/antianginal agents.

Pharmacology
Amlodipine, a dihydropyridine calcium channel blocker, reduces peripheral vascular resistance and blood pressure by inhibiting calcium influx into vascular smooth muscle. It increases coronary blood flow, lowers cardiac afterload, and improves myocardial oxygen supply.

Dosage & Administration

• Hypertension: Start at 5 mg once daily (max 10 mg).

• Angina: 5–10 mg once daily (use 5 mg in elderly/hepatic impairment).

• Special Populations:

  • Hepatic impairment: Initiate at 2.5 mg.

  • Pediatric (6–17 years): Start at 2.5 mg, may increase to 5 mg after 4 weeks.

• Take with or without food.

Interaction

• Avoid with beta-blockers in severe left ventricular dysfunction.

• Minimal interaction with digoxin, cimetidine, warfarin.

• No food effect on absorption.

Contraindications
Hypersensitivity to dihydropyridines, pregnancy.

Side Effects
Common: Dizziness, flushing, headache, peripheral edema. Serious: Hypotension, paradoxical angina, rash, liver dysfunction.

Pregnancy & Lactation
Category C: Use only if benefits outweigh fetal risks. Discontinue breastfeeding.

Precautions & Warnings

• Hepatic impairment: Start at 2.5 mg; titrate slowly.

• Children <6 years: Safety not established.

• Renal impairment: No dose adjustment needed (not dialyzable).

Overdose Effects
Severe hypotension, reflex tachycardia, shock. Manage with IV fluids, vasoconstrictors, calcium gluconate, and gastric lavage.
