Indications: Amaryl M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus whentreatment with both glimepiride and metformin is appropriate.This combination is used in patients whose blood sugar is not adequately controlled with diet and exercise alone or with monotherapy of either glimepiride or metformin. It can also be used as replacement for combination therapy of glimepiride and metformin as separate tablets.
Dosage and Administration: The dosage of Amaryl M should be governed by the desired blood glucose level. The dosage must be the lowest which is sufficient to achieve the desired metabolic control. During treatment, glucose levels in blood and urine must be measured regularly. Amaryl M is usually taken once or twice daily with meals. The starting dose is usually one tablet of Amaryl M 1/500 mg or 2/500 mg once daily with breakfast or the first main meal. Dosage adjustments should be made based on blood glucose response. The maximum recommended daily dose of glimepiride is 8 mg and metformin is 2000 mg. Tablets should be swallowed whole and not crushed or chewed.
Side Effects: The most common side effect is hypoglycemia (low blood sugar). Symptoms may include sweating, paleness, tremor, anxiety, unusual tiredness or weakness, confusion, difficulty concentrating, hunger, and visual disturbances. Gastrointestinal side effects associated with metformin, such as nausea, vomiting, diarrhea, and abdominal discomfort, may also occur, particularly at the start of treatment. Other possible side effects include headache, taste disturbances, and changes in blood counts.
Precautions: Amaryl M is contraindicated in patients with type 1 diabetes mellitus, diabetic ketoacidosis, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, and known hypersensitivity to glimepiride, metformin, other sulfonylureas, or other biguanides. It should be used with caution in patients with impaired hepatic function and in elderly or debilitated patients who may be more susceptible to hypoglycemia. Alcohol consumption should be avoided as it can increase the risk of hypoglycemia and lactic acidosis (a rare but serious metabolic complication associated with metformin). Renal function should be assessed before initiating treatment and regularly thereafter.
