Indications
Amaryl 3 mg (glimepiride) is indicated for the management of type 2 diabetes mellitus, particularly when diet, exercise, and weight reduction alone are insufficient. It may be used as monotherapy or combined with insulin/metformin.

Pharmacology
Glimepiride stimulates insulin release from pancreatic β-cells by closing ATP-dependent potassium channels. It enhances peripheral insulin sensitivity and reduces hepatic glucose production.

Dosage & Administration

  • Initial dose: 1–2 mg once daily with breakfast or first main meal.

  • Maintenance: Adjust by 1–2 mg increments every 1–2 weeks based on glycemic response.

  • Maximum dose: 8 mg daily.

  • Amaryl 3 mg: For patients requiring higher doses after titration.
    Individualize dosing; monitor blood glucose regularly.

Interaction

  • Contraindicated: Bosentan.

  • Increased hypoglycemia risk: Insulin, other sulfonylureas, MAO inhibitors, NSAIDs, salicylates.

  • Reduced efficacy: Thiazides, corticosteroids, thyroid hormones, sympathomimetics.

  • Variable effects: Warfarin (monitor INR).

Contraindications

  • Hypersensitivity to sulfonylureas/sulfonamides.

  • Type 1 diabetes, diabetic ketoacidosis.

  • Severe renal/hepatic impairment.

  • Pregnancy/lactation.

Side Effects

  • Common: Hypoglycemia, dizziness, headache, nausea.

  • Serious: Hepatic dysfunction, hemolytic anemia, allergic skin reactions.

Pregnancy & Lactation
Contraindicated. May cause fetal harm. Discontinue if pregnancy occurs.

Precautions

  • Monitor liver/kidney function.

  • Avoid alcohol (disulfiram-like reaction).

  • Risk of hypoglycemia in elderly/malnourished.

Overdose Effects
Severe hypoglycemia (sweating, tremors, coma). Treat with glucose IV.
