Adlinameg 850 

Presentation:Adlinameg 850 Tablet, available in Bangladesh, is a combination tablet containing Linagliptin 2.5 mgand Metformin Hydrochloride BP 850 mg. 

Indications:Adlinameg is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellituswhen treatment with bothlinagliptin and metformin is appropriate.This includes patients whose type 2 diabetes is not adequately controlled on metformin alone, or who are already being treated with separate tablets of linagliptin and metformin.

Dosage and Administration: The dosage of Adlinameg should be individualized based on the patient's current treatment regimen, glycemic control, and tolerability, using the lowest effective dose. It is generally administered twice daily with meals to reduce the gastrointestinal side effects associated with metformin.

  • Patients not currently treated with metformin: The usual starting dose is one tablet of Adlinameg 850 twice daily with meals.
  • Patients switching from separate tablets of linagliptin and metformin: The starting dose should be based on the patient's previous doses of linagliptin (not to exceed 5 mg daily) and metformin.
  • Dosage Titration: Dosage adjustments should be made gradually based on blood glucose response. It is recommended to increase the dose in increments of one tablet twice daily every 1-2 weeks.
  • Maximum Daily Dose: The maximum recommended daily dose of linagliptin is 5 mg and metformin is 2550 mg. With Adlinameg 850, the maximum dose would typically be two tablets twice daily (linagliptin 10 mg and metformin 3400 mg), which exceeds the recommended maximum for linagliptin. Therefore, the maximum recommended dose of Adlinameg 850 is likely one tablet twice daily (linagliptin 5 mg and metformin 1700 mg). Always follow the physician's prescription.
  • Administration: The tablet should be swallowed whole and should not be crushed, split, or chewed.

Side Effects:Common side effects associated with metformin include gastrointestinal disturbances such as diarrhea, nausea, vomiting, andflatulence, particularly during initiation of therapy.These effects are often transient. Hypoglycemia is not usually seen with metformin or linagliptin monotherapy but can occur when Adlinameg is used in combination with insulin secretagogues (e.g., sulfonylureas).Side effects potentially related to linagliptin include upper respiratory tract infection, nasopharyngitis, and headache.Pancreatitis has been reported in some patients receiving linagliptin.Less common side effects may include dizziness and skin rash. Lactic acidosis is a rare but serious side effect associated with metformin.

Precautions:Adlinameg is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis (including diabetic ketoacidosis), or a history of serious hypersensitivity reaction to linagliptin or metformin.It is not recommended for use in patients with type 1 diabetes mellitus. Caution should be exercised in patients with moderate renal impairment (eGFR 30-45 mL/min/1.73 m²), heart failure, liver disease, and excessive alcohol intake due to the increased risk of lactic acidosis associated with metformin. Renal function should be assessed prior to initiation of therapy and periodically thereafter. Discontinue Adlinameg if pancreatitis is suspected.

Storage:Store Adlinameg 850 tablets in a cool (below 30°C) and dry place, protected from light and moisture.Keep out of reach of children.
