Xyril Hydrochloride (Hydroxyzine): A Sedating Antihistamine
Xyril Hydrochloride contains Hydroxyzine Hydrochloride, an anxiolytic antihistamine of the piperazine class. It is indicated for the symptomatic relief of anxiety and tension, for the management of pruritus (itching) due to allergic conditions, and as a sedative for premedication and following general anesthesia. Its effectiveness for long-term use for anxiety has not been established. Xyril belongs to the therapeutic class of Sedating Anti-histamines.
How Xyril Works (Pharmacology)
Hydroxyzine's mechanism of action is primarily to block histamine receptors and suppress activity in the central nervous system:
- H1 Receptor Antagonist: It acts as an H1 receptor antagonist, which is responsible for mediating allergic reactions and causing itching.
- CNS Suppression: It is not a cortical depressant but suppresses activity in certain subcortical areas of the central nervous system, leading to its anxiolytic and sedative effects.
- Additional Effects: It has also been shown to have skeletal muscle relaxation, bronchodilator, antiemetic, and analgesic effects.
- Rapid Absorption: It is rapidly absorbed from the gastrointestinal tract, with clinical effects typically seen within 15 to 30 minutes after oral administration.

Xyril is indicated for:
- Symptomatic relief of anxiety and tension associated with psychoneurosis.
- Management of pruritus (itching) due to allergic conditions like chronic urticaria, atopic and contact dermatoses.

Dosage & Administration
Dosage of Xyril varies depending on the condition being treated and the patient's age. It is available in tablet and syrup forms. Always consult a registered physician for specific dosage instructions.
- Anxiety and Tension (Adults): 50-100 mg four times daily.
- Pruritus (Adults): 25 mg three to four times daily.
- Sedation (Adults): 50-100 mg.
- Children (>6 years): 50-100 mg daily in divided doses for anxiety or pruritus.
- Children (<6 years): 50 mg daily in divided doses for anxiety or pruritus.
- Children (Sedation): 0.6 mg/kg of body weight.

Contraindications:

Side Effects:

- Rare: Hypotension, palpitations, arrhythmias, dizziness, confusion, depression, convulsions, and various hypersensitivity reactions.
Pregnancy & Lactation:
- Pregnancy: It is contraindicated in early pregnancy due to a lack of adequate safety data.
- Lactation: It is unknown if the drug is excreted in human milk. It should not be given to nursing mothers.
Precautions & Warnings:
- CNS Depressants: The potentiating action of Xyril on other CNS depressants (e.g., narcotics, barbiturates, alcohol) must be considered. Dosages of other depressants should be reduced if used concurrently.
- Drowsiness: Patients should be warned of the possibility of drowsiness and cautioned against driving or operating dangerous machinery.
- Use in Special Populations:
- Renal impairment: Half of the normal dose should be given.
- Elderly: It is advised to start with half the recommended dose due to the drug's prolonged action in older individuals.
Drug Interactions:
- CNS Depressants: May potentiate the effects of meperidine, barbiturates, and alcohol.
- Monoamine Oxidase Inhibitors: Should be avoided.
- Epinephrine: Should not be used to treat hypotension from an overdose, as Hydroxyzine counteracts its pressor action.
- Digitalis: Xyril does not interfere with the action of digitalis.
Overdose Effects
- The most common sign of overdose is hypersedation.
- Treatment involves inducing vomiting, gastric lavage, and general supportive care.
- Epinephrine should not be used.
- Hemodialysis is unlikely to be of value.
