Fosfomycin Trometamol Overview

Therapeutic Class: Intracellular Antibiotic

Indications

Indicated only for treating uncomplicated urinary tract infections (acute cystitis) in women aged 18 and older caused by susceptible strains of Escherichia coli and Enterococcus faecalis.

Dosage & Administration

• Adult Dose (18+): Single sachet, taken with or without food.

• Preparation: Dissolve the full contents of one sachet into 100 mL of purified water, stir to dissolve completely, and drink immediately.

• Usage Warning: Not recommended for children. Do not take more than one single dose for a single episode; repeated doses do not improve cure rates and increase the risk of side effects.

Contraindications & Warnings

• Contraindications: Known hypersensitivity to fosfomycin, severe renal insufficiency, or patients undergoing hemodialysis.

• Precautions: As with most antibiotics, treatment can alter colon flora, potentially leading to Clostridium difficile-associated diarrhea (CDAD).

Drug Interactions

• Metoclopramide and other medications that increase gastrointestinal motility lower the serum concentration and urinary excretion of fosfomycin.

Side Effects

• Most Common (>1%): Diarrhea (10.4%), headache (10.3%), vaginitis (7.6%), nausea (5.2%), rhinitis (4.5%), back pain (3.0%), and dizziness (2.3%).

• Rare (<1%): Vomiting, constipation, dry mouth, fever, and rash.

Pregnancy & Lactation

• Pregnancy Category B: Use during pregnancy only if clearly needed and benefits outweigh risks.

• Consult a physician to decide whether to stop breastfeeding or withhold the medication.

Storage

Store in a dry place below 25°C, protected from light. Keep out of reach of children.
