Topirva XR 100

Description

Topirva XR 100 contains topiramate in an extended-release capsule form.

Indications

Topirva XR 100 is indicated for:

• Prophylaxis of migraine headache.

• Adjunctive therapy for partial-onset or primary generalized tonic-clonic seizures in adults and children 2 years and older.

Dosage & Administration

Dosage varies; consult a physician for individualized dosing.

• Migraine Prophylaxis:

  • Adults: Start with 25 mg once daily, and increase weekly to 100 mg once daily.

• Epilepsy (Adjunctive Therapy):

  • Adults: Start with 25-50 mg daily, and titrate to an effective dose.

  • Children (2-16 years): Start with 25 mg daily (or 1 to 3 mg/kg/day), and titrate to an effective dose.

Topirva XR 100 is taken orally, once daily, and should be swallowed whole.

Contraindications

Topirva XR 100 is contraindicated in patients with known hypersensitivity to topiramate or any inactive ingredients.

Precautions & Warnings

• Topiramate can cause metabolic acidosis.

• Decreased sweating and increased body temperature have been reported, especially in children.

• Topiramate may cause cognitive dysfunction and psychiatric/behavioral disturbances.

• Topiramate has been associated with acute myopia and secondary angle closure glaucoma.

• Topiramate can cause fetal harm.

• Increased risk of kidney stone formation.

• Antiepileptic drugs, including topiramate, increase the risk of suicidal thoughts or behavior.

• Topiramate can cause hyperammonemia, sometimes associated with encephalopathy.

Side Effects

Common side effects include paresthesia, fatigue, nausea, dizziness, nervousness, weight loss, and difficulty with concentration/attention, and memory problems.

Less common side effects include depression, anxiety, speech problems, tremor, and vision problems.

Drug Interactions

Topiramate interacts with:

• Other CNS depressants: Increased sedation.

• Oral contraceptives: Reduced effectiveness.

• Carbonic anhydrase inhibitors: Increased risk of kidney stones.

Use in Pregnancy & Lactation

• Pregnancy: Topiramate can cause fetal harm; use only if the potential benefit justifies the fetal risk.

• Lactation: Topiramate is excreted in human milk; consider the potential risk to the infant.

Storage

Store in a cool, dry place, protected from light and moisture, and keep out of reach of children.
