Tolema (Tolfenamic Acid): A Non-Steroidal Anti-inflammatory Drug (NSAID)

 

Tolema contains Tolfenamic acid, an NSAID belonging to the fenamate group. It is a potent inhibitor of the cyclooxygenase enzyme, which reduces the synthesis of inflammatory mediators like prostaglandins. This action helps to alleviate pain and inflammation. Tolema is specifically indicated for relieving the pain of migraine headaches and is also recommended as an analgesic for post-operative pain and fever.


 

How Tolema Works (Pharmacology)

 

Tolfenamic acid acts by inhibiting key enzymes and directly antagonizing receptors:

  • Inhibits Cyclooxygenase (COX): It is a potent inhibitor of the cyclooxygenase enzyme, which is responsible for producing inflammatory mediators such as thromboxane and prostaglandins.
  • Reduces Pain and Inflammation: By inhibiting prostaglandin synthesis, it reduces the pain, swelling, and inflammation associated with various conditions.
  • Direct Antagonism: Tolfenamic acid also has a direct antagonistic action on prostaglandin receptors, providing an additional mechanism for its analgesic and anti-inflammatory effects.
  • Pharmacokinetics: It is readily absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 60-90 minutes. It has a plasma half-life of 2 hours and is metabolized in the liver, with most of it excreted in the urine.

Dosage & Administration

 

Tolema should be taken with food and a glass of water. Always consult a registered physician for specific dosage instructions.

  • Acute migraine attacks (Adults): 200 mg when symptoms first appear, which may be repeated once after 1-2 hours if needed.
  • Mild to moderate pain (Adults): 100-200 mg three times daily.
  • Renal impairment: Dose adjustments may be necessary. It should be avoided in severe renal impairment.
  • Children: A specific pediatric dosage has not been established.

 

Important Considerations & Warnings

Contraindications:

  • Active gastrointestinal bleeding or ulceration.
  • History of gastrointestinal bleeding or perforation related to previous NSAID therapy.
  • Severe heart failure.

Side Effects:

  • Gastrointestinal: Nausea, diarrhea, gastrointestinal discomfort, and bleeding.
  • CNS: Headache, dizziness, drowsiness, visual disturbances, and vertigo.
  • Rare but serious: Angioedema, blood disorders, hepatic damage, pancreatitis, and Stevens-Johnson syndrome.

Pregnancy & Lactation:

  • Pregnancy: Not recommended for use during pregnancy, especially during the third trimester.
  • Lactation: NSAIDs should be avoided if possible when breastfeeding, as they can appear in breast milk in very low concentrations.

Precautions & Warnings:

  • Precaution: Use with caution in patients with asthma, bleeding disorders, cardiovascular diseases, peptic ulceration, hypertension, liver infection, or impaired cardiac or renal function.
  • Dysuria: Increased water intake or dose reduction may help to reduce painful urination.

Drug Interactions:

  • Anticoagulants and other NSAIDs: Increase the risk of bleeding.
  • Antihypertensives: Decreases the antihypertensive response of loop diuretics, beta-blockers, and ACE inhibitors.
  • Other drugs: Increases plasma concentrations of Lithium, Methotrexate, and cardiac glycosides. It also increases the risk of nephrotoxicity with ACE inhibitors, Ciclosporin, Tacrolimus, or diuretics.

 

Overdose Effects

 

  • Symptoms: Headache, nausea, vomiting, gastrointestinal bleeding, drowsiness, dizziness, and occasionally convulsions.
  • Serious poisoning: Acute renal failure and liver damage are possible.
  • Treatment: Patients should be treated symptomatically.

Store in a cool and dry place, away from light. Keep out of the reach of children.
