Tinium 50

Presentation:

Tinium 50 contains 50 mg of Sitagliptin as its active ingredient. It is typically presented as light brown, round, film-coated tablets, often engraved with the dosage strength on one side. The tablets are supplied in blister packs.

Indications:

Tinium 50 (Sitagliptin) is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It isused as:

  • Monotherapy: When diet and exercise alone do not provide adequate glycemic control and metformin is inappropriate due to contraindications or intolerance.
  • Combination Therapy:
    • With metformin, a sulfonylurea, or a PPAR$\gamma$ agonist (e.g., thiazolidinedione) when diet and exercise plus the single agent do not provide adequate glycemic control.
    • As triple combination therapy with metformin and a sulfonylurea, or with metformin and a PPAR$\gamma$ agonist.
    • With insulin (with or without metformin).

Tinium 50 works by increasing the levels of active incretin hormones, which play a role in regulating glucose levels.

Dosage and Administration:

The recommended dosage of Tinium 50 is 50 mg once daily. It can be taken with or without food. The dosage may be adjusted by your doctor based on individual response and renal function.

  • For patients with mild renal impairment (creatinine clearance 50 to <80 mL/min), no dosage adjustment is required.
  • For patients with moderate renal impairment (creatinine clearance 30 to <50 mL/min), the dosage is 50 mg once daily.  
  • For patients with severe renal impairment (creatinine clearance <30 mL/min) or end-stage renal disease (ESRD) requiringhemodialysis or peritoneal dialysis, the dosage is 25 mg once daily.

It is crucial to take Tinium 50 exactly as prescribed by your doctor. Do not stop taking it without consulting your doctor, even if you feel well.

Side Effects:

Commonly reported side effects of Tinium 50 are generally mild and may include:

  • Upper respiratory tract infection
  • Nasopharyngitis (sore throat, runny nose)
  • Headache

Less common or rare but potentially serious side effects include:

  • Hypoglycemia (low blood sugar), particularly when used in combination with a sulfonylurea or insulin.
  • Pancreatitis (inflammation of the pancreas), which can be severe and potentially fatal. Symptoms include severe, persistent abdominal pain that may radiate to the back, with or without vomiting.
  • Hypersensitivity reactions (e.g., anaphylaxis, angioedema, rash, urticaria, cutaneous vasculitis, exfoliative skin conditions including Stevens-Johnson syndrome).  
  • Arthralgia (joint pain), which can be severe.
  • Bullous pemphigoid (a skin blistering condition).
  • Renal impairment.

If you experience any severe or persistent side effects, or symptoms suggestive of pancreatitis or a serious allergic reaction, seek immediate medical attention.

Precautions:

Before taking Tinium 50, inform your doctor if you have:

  • A history of pancreatitis.
  • Moderate to severe kidney disease or are on dialysis.
  • A history of allergic reactions to sitagliptin or any other components of the tablet.  
  • Type 1 diabetes or diabetic ketoacidosis (Tinium 50 is not indicated for these conditions).  

Monitor for symptoms of pancreatitis (severe abdominal pain, vomiting) and hypersensitivity reactions. Patients on combination therapy with a sulfonylurea or insulin may require a lower dose of the sulfonylurea or insulin to minimize the risk of hypoglycemia.

Storage:

Store Tinium 50 tablets below 30°C, in a dry place, and protected from light. Keep out of the reach and sight of children. Do not use this medicine after the expiry datewhich is stated on the carton and blister.
