Sitomet 50/500

Presentation: Each tablet of Sitomet 50/500 contains 50 mg of sitagliptin phosphate monohydrate and 500 mg of metformin hydrochloride. 

Indications: Sitomet 50/500 is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin is appropriate.  

Dosage and Administration:  The usual starting dose is one tablet twice daily with meals, with gradual dose escalation to reduce gastrointestinal side effects associated with metformin. The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommended daily dose of 100 mg sitagliptin and 2000 mg metformin. For patients not currently treated with metformin, the recommended starting dose is 50 mg sitagliptin/500 mg metformin hydrochloride twice daily. For patients already on metformin, the starting dose of Sitomet should provide 50 mg sitagliptin twice daily (100 mg total daily dose) and continue the current metformin dose.  

Side Effects: Common side effects may include diarrhea, upper respiratory tract infection, headache, nausea, and hypoglycemia (when used in combination with insulin or sulfonylureas). Metformin component may cause gastrointestinal disturbances such as nausea, vomiting, and abdominal discomfort, particularly at the start of treatment. Lactic acidosis is a rare but serious potential side effect associated with metformin.  

Precautions: Sitomet 50/500 is contraindicated in patients with hypersensitivity to sitagliptin, metformin, or any of the excipients, severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute or chronic metabolic acidosis, including diabetic ketoacidosis, and conditions associated with tissue hypoxia. Caution should be exercised in patients with moderate renal impairment, heart failure, and hepatic impairment. Alcohol consumption should be avoided. Renal function should be assessed before initiating Sitomet and regularly thereafter.

Storage: Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
