Presentation:

Sitomet 50/1000 is presented as film-coated tablets for oral administration, containing 50 mg of sitagliptin and 1000 mg of metformin hydrochloride. Specific packaging details are available on the product.

Indications:

Sitomet 50/1000 is indicated in adults with type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control when combination therapy with sitagliptin and metformin is appropriate.

Dosage and Administration:

The dosage is individualized based on the patient's current regimen, efficacy, and tolerability, not exceeding the maximum daily doses of 100 mg sitagliptin and 2000 mg metformin. Typically administered twice daily with meals. Swallow tablets whole; do not crush or chew. Gradual dose titration is recommended to minimize metformin-associated gastrointestinal effects. Renal function should be evaluated before initiating and periodically during treatment.

Side Effects:

Common side effects may include gastrointestinal disturbances (diarrhea, nausea, vomiting, abdominal discomfort), headache, and upper respiratory tract infections. Hypoglycemia risk may increase with concurrent use of insulin or sulfonylureas. Lactic acidosis is a rare but serious potential side effect associated with metformin.

Precautions:

Assess renal function prior to initiation and regularly thereafter; contraindicated in severe renal impairment (eGFR < 30 mL/min/1.73 m²). Use caution in patients with hepatic impairment, heart failure, and excessive alcohol intake. Temporary discontinuation may be needed for surgical procedures or iodinated contrast imaging. Monitor for signs of pancreatitis and allergic reactions.
