Sergel MUPS (Esomeprazole)

Indications

Sergel MUPS, a Proton Pump Inhibitor (PPI), is indicated for conditions related to excess stomach acid:

• Gastro-esophageal Reflux Disease ($\text{GERD}$), including healing and maintenance of erosive esophagitis.

• Risk reduction for gastric ulcers associated with $\text{NSAID}$ use.

• H. pylori eradication (as part of triple therapy).

• Zollinger-Ellison syndrome and idiopathic hypersecretion.

Pharmacology & Formulation

Esomeprazole works by blocking the proton pump (the hydrogen-potassium ATPase enzyme system) in the stomach's parietal cells, which effectively inhibits the secretion of gastric acid.

The drug uses $\text{MUPS}$ (Multiple-Unit Pellet System) technology, which combines the advantages of tablets and pellet-filled capsules. This technology ensures greater bioavailability, uniform drug release, and minimizes localized irritation and the possibility of "dose dumping" in the stomach.

Dosage (Adults $\ge 18$ years)

Administration

The tablets must be swallowed whole with liquid; do not chew or crush them. If necessary, you can disperse the tablet in half a glass of non-carbonated water and drink the liquid with the pellets immediately. Do not chew the pellets.

Contraindications & Warnings

• Contraindication: Known hypersensitivity to any part of the formulation.

• Precautions: Use carefully if the patient has severe liver dysfunction or severe renal impairment.

• Fracture Risk: Taking a like Sergel {MUPS} for more than one year may slightly increase the risk of hip, wrist, and spine fracture.

• Side Effects: Most common events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth.

• Drug Interactions: Sergel {MUPS} inhibits gastric acid secretion, which may interfere with the absorption of drugs that require an acidic  (e.g., ketoconazole, iron salts, and digoxin).

Pregnancy & Lactation: The manufacturer advises caution, as it is unknown if Esomeprazole is excreted in human breast milk.
