Indications

Rupa-Aid (Rupatadine) is a non-sedating antihistamine indicated for the symptomatic treatment of:

  • Seasonal & Perennial Allergic Rhinitis: To relieve symptoms of hay fever and year-round allergies.
  • Urticaria: To treat hives.

 

Pharmacology

Rupatadine is a long-acting, selective H1-histamine receptor antagonist. It also has the unique ability to antagonize the platelet-activating factor (PAF). Both histamine and PAF are key mediators of inflammation and allergic reactions. By blocking both, Rupatadine provides a more comprehensive anti-allergic effect. It also inhibits the degranulation of mast cells and the release of pro-inflammatory cytokines like TNF-alpha.


 

Dosage & Administration

  • Adults and adolescents (>12 years): 10 mg (one tablet) once daily. It can be taken with or without food.
  • Children 2-11 years (Oral Solution):
    • Weighing 25 kg or more: 5 ml once daily.
    • Weighing 10 kg to less than 25 kg: 2.5 ml once daily.

 

Interaction

  • Ketoconazole or Erythromycin: Co-administration can increase the systemic exposure of Rupatadine. Use with caution.
  • Statins, CNS depressants, or Alcohol: Use with caution when taken together, as Rupatadine may enhance their effects.
  • Grapefruit/Grapefruit juice: Should not be consumed simultaneously with Rupatadine.

 

Contraindications

Rupa-Aid is contraindicated in patients with a known hypersensitivity to Rupatadine or any of its ingredients.


 

Side Effects

  • Common: Drowsiness, dizziness, and asthenia (weakness).
  • Uncommon: Increased appetite, joint pain, back pain, cough, diarrhea, dry throat, nasal dryness, nausea, vomiting, and rash.
  • Rare: Palpitations and tachycardia (rapid heartbeat).

 

Pregnancy & Lactation

There is no clinical data on the use of Rupatadine during pregnancy. It should not be used by pregnant or nursing women unless the potential benefit outweighs the potential risk.


 

Precautions & Warnings

  • Use with caution in patients with a known history of prolonged QT interval, uncorrected low potassium levels, or proarrhythmic conditions (like clinically significant bradycardia or acute myocardial ischemia).
  • Caution should be exercised in elderly patients (65 years and older).
  • Use is not recommended for patients with impaired kidney or liver function, as there is no relevant clinical data available.
